NOTE - This is a REMOTE PART TIME position at 20 hours/week.

What you’ll be responsible for

• Communicate with patients to understand their case and provide education to help improve outcomes
• Maintain timely accurate entry of patient interactions in the EMR
• Monitor patient progress to ensure adherence and success
• Identify additional resources and interventions that will help patients successfully improve their condition
• Review medical records to identify patients who would benefit from additional care
• Remote assessment, advice, education, and triage of patients under our care

This may be a fit if you have

• Experience providing telemedicine care to patients
• Outstanding communication skills, including the ability to be personable and supportive of patients and communicate medical information to non-medical teams
• Experience leading and mentoring non-licensed team members
• Passion for innovation and process improvement
• Both pediatric and adult experience is strongly preferred
• Proficient computer and internet navigation skills
• Registered Nurse with a background in respiratory care
• Certification or experience as asthma and/or COPD educator is preferred
