JOB SUMMARY
The QMHP is responsible for completing assessments for new and current members to ensure continuity of care and appropriate level of care to ensure compliance with funders. QMHP will work with members using a trauma informed client centered approach, through therapeutic services until linkage to permanent service delivery is completed.
JOB DESCRIPTION ROLE AND RESPONSIBILITES

  • Provide case consultation and coordination of care as needed
  • Develop individualized treatment plans with the input of the client, program staff, directors, and other relevant providers when appropriate
  • Ensure warm hand-off to assigned program staff after all initial assessments
  • Complete all documentation in EHR within a timely and accurate manner
  • Review consent forms and rights with client prior to starting assessment.
  • Perform other related duties and/or projects as assigned
  • Work flexible hours to accommodate members needs
  • Monitors and resolves failed activities
  • Maintain documentation requirements in compliance with accrediting bodies
  • Conduct comprehensive assessment (IM+CANS) for new members seeking mental health services
  • Assist program staff in completing assessment reviews and updates to ensure compliance with funding sources
  • Collaborates with CCO to develop and maintain training guides

QUALIFICATIONS

  • Master's degree required in psychology, social work, counseling, or related field;
  • Must pass mandatory background checks and pre-employment drug screen.
  • Experience and competence in trauma-informed models and treatment modalities;
  • Excellent time management skills;
  • Ability to handle sensitive and confidential information.
  • Proficiency using Electronic Health Records system (EHR);
  • Experience in working with trauma in children, adolescents and adults;
  • Service oriented, empathetic and able to interact and develop rapport with a diversified population
