Job Description

This position is located in the DC Department of Health, HIV/AIDS, Hepatitis, STD and TB Administration (HAHSTA), Care and Treatment Services Division. The position serves as a Community Health Worker (CHW), which facilitates access to HIV interventions and services to residents of the Washington, DC Eligible Metropolitan Area. This position is part of a partnership with medical providers and community-based organizations. The CHW works as part of an interdisciplinary care management team that aims to improve health outcomes for people with complex health conditions and social contributors to health. The CHW is housed 100% off-site at a community provider site to fully integrate with the medical providers and other community partners to ensure consumers receive culturally appropriate, timely, and coordinated care; and have access to social services and resources to address barriers to good health.

Duties and Responsibilities

The incumbent will provide direct services to include the following: outreach; health education; conduct screening and intakes; make and track referrals; data collection and reporting; development of individualized care plan that includes goal setting, with a particular emphasis on social needs, barriers to care, behavioral health support, medication management, and chronic disease self-management. The incumbent will collaborate with community medical providers to facilitate service delivery that is culturally sensitive and appropriate. In addition, provide comprehensive reports of program results and disseminate results in a wide range of venues including publication summaries and special reports. Participate in the development of goals and objectives for the program functions and overall development.

The incumbent also participates in the development of goals and objects for program functions. Assists in developing and administering the program budget. Assists in the development of grant applications and in the development of procedures and research manuals for the division.

Qualifications and Education

Individuals must possess at least one (1) year of specialized experience must have been equivalent to at least the next lower grade (9) level in the normal line of progression for the occupation in the organization. Specialized experience is defined as experience that is typically in or related to the work of the position to be filled, and has equipped you with the particular knowledge, skills, and abilities to successfully perform the duties of the position, and that is typically in or related to the work of the position to be filled.

Preferred Experience

2 years of experience providing community health services, case management and/or working in a public health or medical setting.

Experience working within HIV related programs.

Experience working with youth (ages 13-24), black women, black men who have sex with men, and/or Latino men who have sex with men.

Experience with community collaboration and partnerships.

Experience with group facilitation and counseling.

Working Conditions / Environment:

The incumbent must be able to carry out official duties in a variety of settings and conditions. Routine work will occur primarily in an office setting.

Tour of Duty: Monday - Friday 8:15am - 4:45pm
