Job Description

We’re looking for a passionate and highly engaged Psychiatric Nurse Practitioner to join the Assertive Community Treatment Team (ACT) as our Medical Director.

Grace Medical Center offers comprehensive outpatient behavioral health services including, adult outpatient services, adult day hospital, child outpatient, psychiatric emergency department services, two substance abuse programs, residential beds for behavioral health patients, psychiatric rehabilitation program, case management program, a community-based behavioral health facility, and an Assertive Community Treatment Team.

ACT is a multi-faceted approach to caring for and treating individuals with a severe mental health diagnoses, and who are unable to utilize traditional OMHC Services to manage their illness. ACT is one of the oldest and most widely researched evidence-based practices for behavioral health. The model is a team-based, comprehensive, person-centered, and recovery-based approach to care for individuals with severe and persistent mental illness.

About the Position:

  • Full census capacity: 115 clients; average is 80 clients
  • Provide patient care to patients 18 years and older
  • Schedule: Monday – Friday – 8:30 am -5 pm. No weekends, but available by phone
  • 80% clinical with 20% administrative oversight of nursing team and the maintenance of ACT and CARF accreditation.
  • The program includes: 1 physician or 1 nurse practitioner, and 10 support staff including a dynamic supervisor who is an experienced, licensed clinician and handles the day-to-day administrative aspects of the ACT Fidelity and CARF accredited program. There are also 3 FTE nurses who have experience working with this population of patients
  • Telehealth services can also be provided per the state of Maryland and Fidelity requirements.

Specific Job Duties:

  • Coordinates with team Supervisor clinically and administratively
  • Helps to ensure that the ACT team maintains Evidence Based Practice status as an ACT team and, passes CARF accreditation
  • Supervise the nurses on the team per ACT Fidelity and CARF requirements
  • Provide team trainings including for Suicide prevention, and Trauma Informed Care
  • Goes out in the community to see patients per ACT Fidelity model requirements
  • Ensure that clients are seen by Team Psychiatrist/NP at least once every two months, ideally monthly

If you possess the following qualifications, please apply immediately:

  • Demonstrated computer proficiency
  • Excellent communication skills and the ability to work well in a team environment
  • Ability to communicate effectively, both in writing and verbally
  • Master’s degree (DNP/ DNS or eligible preferred) and hold certification from the Maryland Board of Nursing to practice as an Advanced Practice Nurse, (APRN, or CRNP/PMH-BC)
  • Maryland CDS and DEA license
  • Experience working as a NP in an OP psychiatric setting

What is Assertive Community Treatment (ACT)?
Services provided by ACT include nursing, substance use disorder services, case management, psychiatric medication and therapies, employment, peer support, and more. These services are provided in the homes or communities of those being served.

Assertive Community Treatment teams have a low client to staff ratio and are available to clients 24/7. ACT is characterized by (1) low client to staff ratios (no more than 10 clients per staff member); (2) providing services in the community; (3) shared caseloads among team members; (4) 24-hour availability of the team, (5) direct provision of all services by the team rather than referral; and (6) time-unlimited services.

The ACT team staff are highly dedicated, passionate about serving those living with severe mental illness, and collaborate and support each other in this intensive work. Most staff members have been part of this dynamic team for many years, even decades.
