About you:


The Provider Relations Manager works in collaboration with the Provider Relations representatives, Performances Directors, and practices to identify target opportunities for improved quality, risk adjustment, clinical, documentation workflow, patient experience and performance. We are looking for a proactive, analytical relationship management expert!


Qualifications And Skills


  • Strong interpersonal skills necessary in order develop and maintain collaborative relationships with staff and physicians.
  • Proficient computer skills to include Microsoft Office (Word, Excel, and PowerPoint).
  • High level of verbal and written communication skills with a strong capacity for detail, maintaining clear/effective contact with physicians, employees, administration and corporate personnel
  • Proactive, analytical relationship management expert
  • Strong analytical, critical thinking and problem solving skills to ensure process improvement and projects are completed successfully
  • Minimum of three (3)– years of provider network operational experience
  • Experience working with VBCs (value based contracts)
  • Strong organizational and planning skills to manage multiple priorities and meet required deadlines


Essential Duties And Responsibilities
  • Follow-up on presenting or gathering data or information from the practices
  • Requires local travel; Provider Performance Leads shall at the direction of the senior-most Regional Provider Relations& Performance representative to visit Priority practices and emerging opportunity practices weekly.
  • Information
  • At the direction of PR&P Director facilitates on-site visits to Key Accounts and Priority Practices.
  • Responsible for monitoring provider performance and reporting to Provider Relations and Performance Director to track and share data with practice on performance against goals, escalating any issues in a timely manner. Works collaboratively with Performance staff, Office Managers, and practice staff to coordinate improvement efforts related to overall performance. Implements performance improvement plans at the practice level. Review EMR notes and provide information at the point-of-care to the providers via the EMR. Report on quality measures and evaluate strengths and weaknesses and address workflow deficiencies in collaboration with the CareMax Quality team. Update the quality database based on information in the EMR and closure of quality gaps.
  • Works with Provider Relations& Performance representatives, in collaboration with National VP of sales to identify MA conversion targets and to facilitate on-site market and educational events.
  • Produces and maintains record by keeping documentation up to date and reporting progress on a weekly basis. Provides dashboard reports to Provider Relations and Performance representatives and assigned practices including recommendations on improvement.
  • Workflows in place for prioritized practices
  • Educates providers and leadership on clinical indicators to capture the severity of illness of the patient. Develops and delivers training materials (Provider and Practice-Facing). Provides leadership and staff with training, on-site collaboration and on-going performance reporting.
  • Coordinate with Provider Relations Director for on-site visits to Key Accounts and Priority Acts as performance-lead, working in collaboration with Provider Relations and Performance Directors and practices to identify target opportunities for improved quality, risk adjustment, clinical and documentation workflow, patient experience and performance. Identify opportunities for workflow improvement and is responsible for developing and implementing process improvement plans to enhance provider and staff workflow.
  • Creation of practice“Playbooks”
  • Documentation of all communication and training
