Peaceful Living is seeking a Program Specialist to provide and promote for the welfare, well-being, the care and programmatic needs of individuals that we support. The primary focus will be to coordinate and complete each assessment, participate in the development of the Individual Support Plan (ISP), and implement the ISP as written, reviewing all relevant program documentation monthly and coordinating services provided to each person on their caseload, up to a maximum of 30 individuals, ensuring measurable progress of outcomes identified and active treatment.
Essential Job Functions
  • Reviews ISP with the individual and documents the review accordingly. Provides documentation of review to the SC and team members, providing the option to decline the ISP review documentation, as per regulatory guidelines.
  • Communicates all changes related to the individuals' needs to the SC, and all other team members as applicable and provides onsite training as needed.
  • Oversees the implementation of the ISP as written, by supervising, monitoring and evaluating services provided to the individual.
  • Coordinates the training of Residential Companions, in the content of health and safety needs relevant to each individual and the outcomes and services identified in their ISP.
  • In conjunction with county and ODP works to ensure proper billing, including rep payee status (upon admission and ongoing), SSI, SSDI and all other funding sources for each person on their caseload.
  • Oversees all new admissions, including but not limited to: coordinating transitional visits, all medical and dental exams and other documentation needed prior to admission to ensure regulatory compliance.
  • Fulfills the role of "plan lead", relating to development, annual update and revision of the ISP; and ISP review and revision. Attends the ISP meetings for each individual on their caseload
  • Works closely with county and ODP to re-evaluate the needs of each person and communicate changes to the level of care in a timely fashion, to ensure the quality of care and proper billing.
  • In conjunction with the support team participates in the development of the ISP, ISP annual update and ISP revision
  • Coordinates and complete assessment for each individual, as required by regulation.
  • Coordinates all services (including behavioral supports) provided to the individual in alignment with their ISP and recommends revisions to services or outcomes identified in the ISP, as needed, to the SC and team members.
  • After ISP is created by Supports Coordinator (SC), reviews the ISP, annual updates and revisions for content accuracy. Reporting content discrepancies to the SC and other team members, as applicable.
  • Develops and oversees the proper documentation of program goals and outcomes identified in ISP, by reviewing, signing and dating the monthly documentation of an individual's participation and progress toward outcomes, and ensure active treatment.
Education/Experience
A master's degree or above from an accredited college or university and 1-year work experience working directly with individuals with an intellectual disability. A bachelor's degree from an accredited college or university and 2 years work experience working directly with individuals with an intellectual disability; associate's degree or 60 credit hours from an accredited college or university and 4 years work experience working directly with individuals with an intellectual disability.
