Summary:

  • Responsible for discharge planning and contributing to the patient's treatment plan through and understanding of social, psychological and/or environmental needs related to the impact of illness and need for hospitalization. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.

Responsibilities:

  • Coordinates the discharge planning process to ensure a safe and timely transition from hospital to home, community, or institutional setting.
  • Facilitates legal matters, financial entitlements and complies with grants and contracts when applicable.
  • Provides counseling to patients, families and groups.
  • Collaborates and cooperates with other members of the interdisciplinary health care team to ensure continuity of care.
  • Responsible for documentation in the medical record, statistics and reports as required.
  • Enhances own professional development and maintains formal and informal continuing education for self improvement.
  • Uses effective interpersonal skills in all interactions.
  • Maintains relationships with community agencies and keeps up-to-date on resources.
  • Identifies and attends to the psychological needs of patients and families.

Credentials:


Education or Equivalent Experience:

  • Master of Arts or Science (Required)
  • Social Work 0-1 years Health Care experience in field instruction and/or employment in a health care setting.
