Job Definition
SCOPE OF POSITION
The position description is only to be used as a guide. It does not constitute any type of contract between the agency and employee. At the agency’s discretion, the description may be changed and additional duties, qualifications or requirements may be added at any time. This position will demonstrate competency in the care of the (circle all that apply): Neonate, Infant, Pediatric, Adolescent, Adult, Geriatric patient population.
The Physical Therapist (PT) has the responsibility to admit, evaluate, develop the plan of treatment and revise if necessary, prepare progress reports, and plan for discharge of patients according to physician orders. The PT works in conjunction with other team professionals, patients and their families to provide patient care and education as well as participates in in-services programs. The PT will have good documentation skills and succinctly document patients status and develop measurable skills. The PT will supervise PTAs and may be asked to assist with orientation/training of new employees and students.
The PT should have the ability to provide services to pediatrics, adolescents, adult and geriatric patients; must be able to assess and interpret data about patient’s age-specific needs and provide appropriate care to patient group.
The PT will also carry out other departmental duties as assigned. The PT must have the ability to work cooperatively with physicians, other staff members, patients and their families with dignity, courtesy, and respect. The PT must show initiative and good judgment, be outgoing and flexible to adjust to all possible situations and have flexibility in hours to meet the needs of HomeCare and Hospice patients and to participate in weekend rotations as needed. The PT must pay close attention to detail and provide superior levels of care. The PT should be committed to continuing education, upgrading his/her skills, and be willing to cross train in other areas as necessary.
This position may be required to cross train within the agency. Responsibilities in secondary position may require access to medical records and other computer access exceeding routine computer usage. Access to patient information may be necessary to fulfill the requirements of this role; therefore, adherence to Ballad Health privacy and security policies and practices is a must.

EDUCATION AND EXPERIENCE
Must be a graduate of an accredited Physical Therapy Program. One to three years’ experience in health care setting is preferred. Must have knowledge of theory, techniques and practices of the Physical Therapy profession as well as of the use, care and maintenance of professional Physical Therapy equipment. Must successfully complete agency orientation. Must have dependable transportation. Must carry current personal collision and liability insurance on any vehicle driven. Must maintain current driver’s license in the appropriate state.
Must carry current license to practice Physical Therapy in the appropriate state.
Membership in the American Physical Therapy Association is desirable but not mandatory.
Must possess and maintain current certification in cardiopulmonary resuscitation.
