Job Description

Job Title: Pharmacist Job Summary:

Provides proper and adequate pharmaceutical service, including preparation and dispensation of all drugs. The Staff Pharmacist ensures proper storage, packaging, stability, and disposition of pharmaceuticals.

Experience, Education, Training, Special Skills, and Licensure:

  • Bachelor's degree
  • Licensed as a Registered Pharmacist in Louisiana

Job Function:

  • Reviews patient information to evaluate appropriateness of drug therapy and dosing regimens as well as absence of contraindications including but not limited to drug-drug, drug-food, drug-laboratory, and drug disease interactions and communicates clinically relevant interactions to prescribers and department staff as appropriate. Recommends drug discontinuation or dosage alteration when indicated.
  • Assist the medical staff and department staff in selecting therapy regimens which are safe, cost effective, and meet individual patient needs. Suggest non-drug therapy when appropriate. Suggest non formulary drug therapy when appropriate.
  • Pharmaceutical Care

    • Serves as a liaison to nursing staff by completing routine rounds with unit leaders or serves on and/or participates in committee. Committee participation is not limited to the pharmacy department alone.
