Job Title: Patient Navigator

Primary Purpose


The Patient Navigator role is crucial in SEP’s efforts to provide quality care to our patient population. By working with the various SEP offices and patients directly, Patient Navigators coordinate the care of patients to enable a seamless experience for both the patient experience and provider-office workflows. These workflows are accomplished through centralized telephonic support.


The Patient Navigator’s focus includes but is not limited to the following areas:


  • Post-visit follow-up
  • Order management.
  • Pre-visit patient outreach
  • Appointment scheduling
  • Care-gap outreach and intervention


Fulfilling these responsibilities requires the Patient Navigators to ensure that all pertinent medical information and care needs for patients are identified, documented, and communicated to providers if necessary. The Patient Navigators are responsible for working collaboratively with the Value Based Performance team, Provider practices, and the Clinical Transformation/Primary Care departments.


The Patient Navigator is responsible for promoting a culture of accountability and transparency across all SEP offices.


Education


Minimum: High School Diploma/GED


Desired: Associates degree in Health Services or related discipline.


Licenses And Certifications


Current Driver’s License in good standing and reliable, insured transportation.


Years Of Experience


Preferred: 2 years of experience in medical healthcare setting.

  • Must work carefully and precisely with attention to detail.
  • Excellent verbal and written communication skills.

  • Should attain working knowledge electronic health record applications. Previous use of EPIC preferred.
  • Skills in establishing and maintaining effective working relations with physicians, office employees, and patients.
  • Recommended proficient computer skills especially Microsoft Word, Excel, PowerPoint and Outlook.
