Job Description

Provides support to the Patient Discharge Specialist with the completion of the discharge arrangements required for patients who are transitioning from the hospital to an extended care facility, home with services, or another appropriate level of care. Assists with verification of insurance benefits for coverage of post-discharge coverage of home health services, durable medical equipment, outpatient services, and/or extended care facilities. Assists with obtaining and/or arrange transportation services and authorizations. Ensures final discharge arrangements are documented and communicated to the discharge team, patients, and family.

 

  1. Meets daily with Care Transition colleagues to coordinate arrangements outlined in discharge plan.

 

  1. Communicates with Patient Care Facilitators regarding final arrangements for the discharge plan

 

  1. Assists by calling and/or faxing information to extended care facilities, home health agencies, durable medical equipment agencies, and infusion companies completing appropriate documentation.

 

  1. Assists with referrals to State agencies to be in compliance with completion of nursing home prescreens, criminal background checks and face-to-face documentation for Medicare/Medicaid coverage of services.

  

  1. Obtains insurance verification of benefits for discharge services and/or obtains in-network providers, completing appropriate documentation.

 

  1. Coordinates transportation arrangements, communicates transport times to Patient Care Facilitator/ Social Work staff and patient/ families, and completes appropriate transportation documents.

 

  1. Assists in maintaining nursing home and transportation resource materials.

 

  1. Performs ongoing chart documentation on discharge services that were set up.

 

  1. Participates in departmental on-call and week end coverage schedule.

 

  1. Develops working relationships with outside agencies, hospitals, transportation companies, and providers of needed services.

 

  1. Maintains working relationship with all relevant areas of the hospital.

 

  1. Upholds the mission and vision of MHS.

 

  1. Participates in continuing education and in-service training to support professional growth and maintain expertise in discharge coordination functions.

 

  1. Performs other related work as required or requested.

 

Education:

  • Education equivalent to graduation from high school.

Licensure/Certification/Registry: (preferred not required)

  • Completion of Certified Medical Assistant Program; certification as a Medical Assistant.

Experience:

  • Two years’ experience in medical office setting preferred, with Medical Assistant certification.

     or

  • 5 years as a patient care technician, unit secretary or home care setting.

Other Knowledge/Skills/Abilities:

  • Demonstrates ability to work independently.
  • Understanding of healthcare reimbursement mechanisms preferred.
  • Ability to project a professional, friendly, solution-focused demeanor with strong oral and written communication skills.
  • Electronic health record documentation skills required.
  • Functional office computer, time management and organization skills
