DEPARTMENT SUMMARY: The St Charles Health System Pharmacy provides a broad range of inpatient and outpatient pharmaceutical care services to ensure safe and effective medication use.
POSITION OVERVIEW: The Oral Oncology Clinical Pharmacist provides patient centered pharmaceutical services to improve safety, quality, and access to pharmaceutical care for patients we serve across the continuum of care.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Assists the multidisciplinary oncology health care team with initial oral oncology medication selection, review, procurement, and care coordination.
Ensures appropriate and evidence-based medication therapy for patients with hematology/oncology diagnoses.
Advises clinicians on drug selection based on current clinical guidelines to optimize drug therapy.
Monitors patient treatment (chemotherapy and ancillary) medications for appropriateness, drug interactions, toxicities, and cost effectiveness based on current clinical and institutional guidelines as well as patient-specific criteria. Consults with provider to modify treatment regimens.
Educates patients and their families to ensure their understanding of the goal, administration schedule and toxicity management of chemotherapy treatment.
Participates in the training, education and mentoring of other pharmacists and technicians and precepting of pharmacy students and residents.
Maintains liaison relationships with medical and nursing staff; provides timely information pertaining to pharmaceuticals, state and federal regulations regarding drug controls, and Joint Commission Standards.
Provides supervision of assigned pharmacy technicians engaged in pharmacy practice. Ensures compliance with federal, state, and local laws and regulations and the maintenance of required records.
Extracts pertinent information from patient/other sources for the patient's medical management plan.
Operates pharmacy computer system(s) and other equipment.
Assists with development of collaborative practice agreements, policies, and procedures.

EDUCATION
Required: Bachelor of Science degree in Pharmacy.
Preferred: PharmD., Completion of an ASHP accredited PGY-1 residency training
LICENSURE/CERTIFICATION/REGISTRATION
Required: Current registration by the Oregon Board of Pharmacy.
Preferred: ASHP Board Certified Oncology Pharmacist (BCOP), and/or BCPS

Required: Clinical Pharmacy experience
Preferred: Clinical Oncology Pharmacist experience
PERSONAL PROTECTIVE EQUIPMENT
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
PHYSICAL REQUIREMENTS:
