Position Summary


The Nutritionist Dietician will play a crucial part in promoting healthy eating habits and providing nutritional guidance to slow the progression of our members' chronic conditions and associated co-morbid health issues through personalized recommendations and educational resources.

Responsibilities
  • Collaborate with the Care Management team to empower members with self-management plans through education and support in developing healthy eating habits to enhance their well-being
  • Perform other related duties
  • Actively participate in team meetings, multidisciplinary team meetings, in-service trainings, and regular 1:1 meetings with the a direct manager
  • Implement individualized nutrition therapy and educational programs to promote nutrition, disease management, and health awareness
  • Maintain comprehensive electronic medical records following organizational policies and procedures
  • Offer expert advice on general health and nutrition matters to individuals, helping them adopt healthy eating habits to manage chronic conditions effectively
  • Encourage adherence to provider treatment and Care Management plans of care
  • Overcome barriers hindering lifestyle changes, monitor members' progress, and ensure thorough follow-up
  • Work closely within the multidisciplinary team to promote care coordination and deliver quality member-centered care
  • Foster seamless continuity of care by collaborating with external agencies
Requirements
  • Willingness to obtain additional licenses if required, as applicable
  • On Island residency
  • A minimum of 5 years of experience in a clinical/community setting
  • Certification as a Nutritionist or Registered Dietitian with an active current license, as applicable, is desired
  • Specific experience in managing chronic conditions
  • A degree or certification in foods and nutrition, dietetics, or other health-related fields; a Master's degree is preferred
  • Fluency in English & Spanish
Skills
  • Demonstrate cultural awareness and knowledge
  • Capable of independently identifying problems, clarifying facts, and organizing information to identify trends, information gaps, or the need for further information
  • Excellent written and verbal communication skills
  • Possess strong interpersonal skills to motivate patients and other team members
  • Strong organizational abilities to manage various responsibilities effectively
  • Must have experience in company managed systems (HRIS, proprietary software)
  • Must have experience in virtual meeting applications (Zoom, RingCentral/etc.)
  • Proficiency with Microsoft suite of tools, including Excel, Word, PowerPoint, Access, Project, and Visio
