Overview:
Nurse Practitioner (GS-0610-12) - Naval Medical Center San Diego (NMCSD)

Downtown San Diego is located on San Diego Bay. Balboa Park encompasses several mesas and canyons to the northeast, surrounded by older, dense urban communities including Hillcrest and North Park. To the east and southeast lie City Heights, the College Area, and Southeast San Diego. To the north lies Mission Valley and Interstate 8. The communities north of the valley and freeway, and south of Marine Corps Air Station Mirmar, include Clairemont, Kearny Mesa, Tierrasanta, and Navajo. Stretching north from Miramar are the northern suburbs of Mira Mesa, Scripps Ranch, Rancho Penasquitos, and Ranch Bernado. The far northeast portion of the city encompasses Lake Hodges and the San Pasqual Valley, which holds an agricultural preserve. Carmel Valley and Del Mar Heights occupy the northwest corner of the city. To their south are Torrey Pines State Reserve and the business center of the Golden Triangle. Further south are the beach and coastal communities of La Jolla, Pacific Beach, Mission Beach, and Ocean Beach. Pont Loma occupies the peninsula across San Dego Bay from downtown. The communities of South San Diego (an Enclave), such as San Ysidro and Otay Mesa, are located next to the Mexico-United States border and are physically separated from the rest of the city by the cities of National City and Chula Vista. A narrow strip of land at the bottom of San Diego Bay connects these southern neighborhoods with the rest of the city.

Responsibilities:


Nurse Clinician


  • Plans and coordinates nursing care of the General Surgery Patient and independently solves problems related to nursing care.
  • Defines overall needs of patients admitted and formulates a holistic plan of care based on nursing diagnoses to guide the health care team in meeting those needs.
  • Establishes and monitors established clinical pathways to improve patient care and result in positive outcomes. Applies quality care principles to affect positive change throughout the health care system.
  • Evaluates the appropriateness and effectiveness of treatment regimens in relation to patient response to include procedures, medications, clinical documentation, progress notes, consults, doctor's orders, pharmacy requests, and/or blood bank requisitions.
  • Independently develops and implements therapeutic plans and clinical practice (verbal and written) in collaboration with the attending surgeon to achieve nursing and medical goals. Determines necessity for laboratory studies, coordinate ordering of lab tests and evaluates results. Performs specialized diagnostic and therapeutic procedures.

  • Identifies, co-authors, and implements treatment for the delivery of quality health care to patients and support to their family.
  • Identifies health problems and determines need for patient admission.
Daily conducts rounds on admitted surgical patients to clarify plan of care, patient response to treatment plan and progress, and ascertain the need for additional or modified services.
  • Performs follow on assessment, collaboration, and evaluation of surgical patients after discharge from inpatient care.

  • Assesses and if a need found, develops, implements, and evaluates unit specific continuous improvement projects.
  • Directs emergency interventions and initiates appropriate emergency measures with, or in the absence of medical support.
  • Seeks nursing research and research utilization in the specific care of surgical patients and their families. Collects data relevant to surgical treatment and consult with Nurse Research Leaders and the Command's Clime Research.
  • Acquires health histories, performs physical examinations, diagnoses medical problems, prescribes plan of care, documents patient progress and interventions, records clinical summaries, coordinates health care and manages patients in collaboration with medical provider as appropriate.
  • Provides direct care; stabilizes patients and minimizes complications, provides physical and psychological support and prescribes medications and treatments to facilitate restoration of maximal health potential while evaluating current and future health risk factors.
  • Writes discharge orders, completes and dictates discharge summary, and provides outpatient treatment and follow-up for specialty patient population (discharging privileges).
  • Assesses educational needs and level of understanding of patients and families and determines best approach to fully meet learning requirements through use of appropriate adult training techniques.

  • Develops and implements educational plans for patients and families. Evaluates and maintains effective teaching programs and updated, informational materials. Coordinates patient and family education.


Technical


  • Collaborates with other advanced practice nurses, medical and nursing professionals, mental health, nutritional services, and others involved in providing support to the target populations.
  • Coordinates health care services of surgery patients, both inpatient and outpatient.
  • Contributes to patient education. Provides patient education to patient and family during hospitalization. Provides feedback to pre-operative education team to provide timely, research-based practice guidelines.
  • Utilizes advanced pharmacological knowledge to manage complex medication administration. Monitors overall effect and matches patient's response and physiological parameters with desired effect. Special attention given to post-operative period to assess need for medication dose adjustment.
  • Applies, operates and analyzes data from complex surgical patient care and physiological monitoring equipment. Ensures staff competency with the actions identified in the surgical pathway in application, opcra1ion and data analysis received from actions.
  • Performs ongoing assessments and identifies subtle changes in patient conditions for medical diagnosis and treatment. Discriminate between normal and abnormal clinical findings related to surgery performed and initiates interventions to prevent serious physical or emotional problems.

Administrative/Leadership

  • Applies use of computer systems and databases in the provision of patient care, physiological monitoring, and documentation to include, is not limited to the Internet, Microsoft Office (Power Point, Excel, Access, Word), GENESIS (medical record).
  • Provides medical residents and interns with an experienced resource to address questions of management of patient care problems unique to specialty area.
  • Functions as administrative expert for patient care programs. Develops objectives, reviews content and materials, updates programs and content as necessary, monitors presentations, and evaluates patient and staff learning outcomes.
  • Role model for interdisciplinary cooperation and collaboration. Coordinates a team approach in the management of complex patients. Consults with other health care providers and community resources in the coordination and delivery of patient care services across the outpatient care continuum.
  • Participates in problem solving as a recognized nurse leader. Develops consultative services and creates innovative healthcare services to meet specialized needs of target patient populations. Acts as consultant to, and resource for, patients, families, nurses, and other health care professionals in clinical areas.
  • Oversees professional development and clinical competency of nursing personnel assigned to the Department of General Surgery. Provides input on clinical performance and staff competency.
  • Functions in a collaborative environment involving medical and nursing staff, Psychiatric department, nutritional department, emergency department, social services department, and other health care departments, the patients, their families and community resources.

Performs other duties as assigned.

Qualifications:

Must be a US Citizen

Degree:
A graduate or higher level degree from an accredited professional nursing educational program in Nurse Practitioner is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Graduate with a Master's degree from an accredited university or college as a Family, Emergency or Acute Care Nurse Practitioner or graduate with a Master's degree in a clinical specialty and completion of a post-graduate certificate program as a Family, Emergency, or Acute Care Nurse Practitioner from an accredited program.

Foreign Degree:
A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.

Licensure:
A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.

Licensure:
A current, active, full, and unrestricted license or registration as a professional nurse in the specialty of Family Practice Nurse Practitioner or related specialty from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Must be board eligible or board certified by a national agency with certification as a Family, Emergency or Acute Care Nurse Practitioner.

Professional Nursing Experience
: At least two years of clinical experience in medically associated specialty area.

Basic Life Support (BLS) is required.
