JOB DESCRIPTION
Essential Job Responsibilities/Expectations
Job responsibilities include those listed in competencies document.
2. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co workers in accordance with our Service Excellence Standards and Core Values.
3. Functions as a part of the multidisciplinary Neuroservices team for provision of care to the Neurology, Stroke, Neurosurgical and Neuro-interventional patients.
4. Develops, implements, and maintains care pathways for patients within the neuroscience service line in partnership with the VP of Neurosciences and the CNO across McLeod Health.
5. Works with the Neuroscience and Emergency Department nursing unit directors along with all disciplines dealing with the Neuroscience patients to improve care processes, patient outcomes and quality of care.
6. Is an integral part of the creation and development of new services offered within the Neuroscience service line.
7. Develops and provides patients with comprehensive treatment summaries/survivorship plans of care upon completion of treatment.
8. Serves as liaison between patients and clinical care providers, facilitating communication and coordination of care as needed.
9. Participates in site specific Neuroservices conferences.
10. Promotes the profession of Neuroservices navigation through patient in quality improvement initiatives within the Neuroservices program.
11. Maintains working knowledge of current therapies and treatment details for the specific disease for the Neuroservices population served. Demonstrates competency in, but not limited to the following (refer to McLeod Health Competency Checklist): • Care Coordination • Effective Communication • Professionalism • Patient Education
12. Is knowledgeable of and adheres to DNV Stroke accreditation standards.
13. Participates in Neuroservices Committees as requested.
14. Participates in community events, such as community health fairs, as requested.
15. Completes annual training and annual educational requirements timely.
16. Performs other tasks, duties or responsibilities as assigned or need for the effective, safe and efficient functioning operation of the hospital.

Qualifications/Training:
- Excellent communication skills – verbal and written
- Ability to work independently Licenses/Certifications/Registrations/Education:
- Must possess a valid SC license – RN, BSN
