Job Description

Summary

Job Summary

Serves as a patient advocate, liaison, and advisor/educator to assist patients with navigating the continuum of care from detection of disease through acute care treatment. Provides individualized assistance to patients, families, and caregivers to help overcome barriers and facilitate timely access to quality medical and psychosocial care. Enhances the patient's quality of life, sense of autonomy, and self-determination for managing own health.
Job Responsibilities
  • Serves as a liaison between patient and clinical staff, physicians, managed care companies, and external community resources to coordinate/maximize resources and identify/resolve barriers to the plan of care.
  • Provides educational resources, support, and coordination to assist patients in securing appointments.
  • Rounds on and reviews assigned patients regularly and evaluates patient progress with plan of care.
  • Provides support, counseling, and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
  • Assists patients and families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources. Intervenes as appropriate to advocate for patient and family.
  • Communicates plan of care to patient and family and solicits concerns, questions, and issues for resolution.
Qualifications
  • RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire AND
  • BLS - Certified as a Basic Life Support Healthcare Provider required within 90 days of employment in this position
  • Associate's degree or higher or Diploma in Nursing required upon hire AND
  • A minimum of 4 years' relevant experience required
