Job Description
Responsible for the day to day operations of the clinic(s).


  • Ensures compliance with DMH, Joint Commission, and Medicaid standards
  • Ensure that approvals in ChartMeds are completed and that every 2-hour medication frequency issues have been resolved before the end of the workday
  • Communicates and works directly with ADULT RESIDENTIAL SERVICES to ensure medications are secured and administered appropriately. Ensure ADULT RESIDENTIAL SERVICES knows when medications are discontinued and need to be removed from the group home
  • Provide Medication List to include new, changed, discontinued medications to Integrated Healthcare Pharmacy for every consumer seen in clinic daily
  • Ensure any discontinued medications are discontinued in ChartMeds and notification sent to Integrated Healthcare Pharmacy
  • Ensures compliance with primary health care policies and procedures, as well as those of external regulatory bodies such as HRSA, HIPAA, OSHA, CLIA.
  • Consult with Providers when needed regarding appropriate medications
  • Provide a list of any medication changes made regarding consumers as a result of Labs, crisis, requests from group home staff to Integrated Healthcare Pharmacy daily
  • Ensure narcotic medications are stored and provided to the group homes per policy.
  • Ensure Refill requests are handled daily and appropriate reconciliation made
  • When anticipating hospital discharges work closely with hospital staff to ensure accurate discharge medication list and prescriptions are received prior to full discharge
  • Ensure that Medication list in NextGen is reconciled with MAR in ChartMeds prior to clinic visits with Provider
  • Communicates directly with Integrated Healthcare Pharmacy to ensure delays in starting medications are minimized, to include monitoring status of Prior Authorizations and Patient Assistance Program approval status. Consults with Provider if a delay is anticipated for direction and/or any changes ordered.
  • Receive medications from Integrated Healthcare Pharmacy daily – verify medications to ChartMeds and Provider order in NextGen/Outside prescriptions from other Providers. Make any changes needed regarding dosage, route, type of administration, etc.
  • Ensure any changes made as a result of Provider clinic visit are correctly verified in ChartMeds once medications are received from Integrated Healthcare Pharmacy
  • Research and eliminate any inconsistencies with consumer’s medications

  • Actively participates in AltaPointe Pharmacy and Therapeutics committee


Pharmacist preferred or possess current RN license to practice in the state of Alabama with a minimum of two (2) years Adult Residential Services proven clinical experience with medication reconciliation and/or quality assurance. Already possess or obtain within 6 months of employment
