Entry Requirements
  • Ability to communicate effectively in oral and written English. Computer literacy required.
  • Bachelor’s Degree in Social Work, Psychology, Sociology or related field from a school accredited or approved by the Council on Social Work Education. Current Michigan licensure as a Bachelor’s Social Worker required.
  • Possess current Michigan motor vehicle license, ability to drive a car, and insured vehicle capable of transport to patient homes or other offices under various weather conditions.
  • 2 years’ experience in a health care setting. Home Care or Hospice experience preferred.
  • Meets MHH established requirements for proof of health status. Physically able to lift 35 pounds. Demonstrates ability to move, position and transfer clients utilizing good body mechanics, lifting techniques and/or transfer assistive devices to avoid manipulating more than 35 pounds.
  • Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.
ORGANIZATION
Reports to appropriate manager, clinical supervisor or designee.
Works under the supervision of a qualified licensed Master’s in Social Work.
Works closely with the Home Care/Hospice interdisciplinary teams, physicians, nurses, ancillary staff and other community personnel.
SPECIFIC DUTIES
  • Assess the emotional factors related to illness including identification of family dynamics and communication patterns.
  • Meets established productivity standards.
  • Collaborates with physicians, nurses, and ancillary staff in developing and implementing patient care plans.
  • Provides information regarding preparation of advance directives.
  • Addresses patient/family questions and issues.
  • Uses effective customer service/interpersonal skills at all times.
  • Assesses environmental resources and obstacles to maintaining safety.
  • Participates in quality and performance improvement measures.
  • Provides assistance to other team members in understanding the social, ethical and emotional factors related to health problems.
  • Supports the Mission, Vision and Values of Munson Home Health.
  • Complies with standard precautions, infection control, and safety procedures.
  • Identifies support systems available to reduce stress and facilitates coping with disease processes including end of life care.
  • Records in a timely manner evaluation data, interventions, all patient contacts including phone contacts, and client and family’s response to interventions in the electronic medical record.
  • Performs all other duties and responsibilities as assigned.
  • Completes assessment of the patient’s/care giver’s/families psychosocial status including potential risk of suicide and/or abuse or neglect.
  • Identifies problems and, in collaboration with the patient/family, develops appropriate goals.
  • Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions.
  • Embraces and supports the Performance Improvement philosophy of Munson Healthcare.
  • Participates in effective interdisciplinary plan of care coordination and adherence to plan of care.
  • Identifies the developmental level of patient/family and obstacles to learning or ability to participate in care of patient.
  • Provides social work services including identification of appropriate community resources and crisis intervention counseling while assessing the patient’s/family’s ability to access those resources.
  • Promotes personal and patient safety.
Hospice Addendum
  • Addresses funeral planning issues and transfer of responsibility regarding financial, legal and health care decisions.
  • Assesses need for counseling related to risk assessment for pathological grief.
