Entry Requirements
- Ability to communicate effectively in oral and written English. Computer literacy required.
- Bachelor’s Degree in Social Work, Psychology, Sociology or related field from a school accredited or approved by the Council on Social Work Education. Current Michigan licensure as a Bachelor’s Social Worker required.
- Possess current Michigan motor vehicle license, ability to drive a car, and insured vehicle capable of transport to patient homes or other offices under various weather conditions.
- 2 years’ experience in a health care setting. Home Care or Hospice experience preferred.
- Meets MHH established requirements for proof of health status. Physically able to lift 35 pounds. Demonstrates ability to move, position and transfer clients utilizing good body mechanics, lifting techniques and/or transfer assistive devices to avoid manipulating more than 35 pounds.
- Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.

Works closely with the Home Care/Hospice interdisciplinary teams, physicians, nurses, ancillary staff and other community personnel.
SPECIFIC DUTIES
- Assess the emotional factors related to illness including identification of family dynamics and communication patterns.
- Meets established productivity standards.
- Collaborates with physicians, nurses, and ancillary staff in developing and implementing patient care plans.
- Provides information regarding preparation of advance directives.
- Addresses patient/family questions and issues.
- Uses effective customer service/interpersonal skills at all times.
- Assesses environmental resources and obstacles to maintaining safety.
- Participates in quality and performance improvement measures.
- Provides assistance to other team members in understanding the social, ethical and emotional factors related to health problems.
- Supports the Mission, Vision and Values of Munson Home Health.
- Complies with standard precautions, infection control, and safety procedures.
- Identifies support systems available to reduce stress and facilitates coping with disease processes including end of life care.
- Records in a timely manner evaluation data, interventions, all patient contacts including phone contacts, and client and family’s response to interventions in the electronic medical record.
- Performs all other duties and responsibilities as assigned.
- Completes assessment of the patient’s/care giver’s/families psychosocial status including potential risk of suicide and/or abuse or neglect.
- Identifies problems and, in collaboration with the patient/family, develops appropriate goals.
- Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions.
- Embraces and supports the Performance Improvement philosophy of Munson Healthcare.
- Participates in effective interdisciplinary plan of care coordination and adherence to plan of care.
- Identifies the developmental level of patient/family and obstacles to learning or ability to participate in care of patient.
- Provides social work services including identification of appropriate community resources and crisis intervention counseling while assessing the patient’s/family’s ability to access those resources.
- Promotes personal and patient safety.
- Addresses funeral planning issues and transfer of responsibility regarding financial, legal and health care decisions.
- Assesses need for counseling related to risk assessment for pathological grief.
