Job Description

Full-Time - MDS Coordinator

Seeking a new nursing career opportunity in a facility that prioritizes a home setting? The Bradley Home is a 30-bed skilled facility located in Meriden, CT, seeking an MDS Coordinator to join our team. Our team consists of compassionate professionals that honor our mission to provide loving care to aged and indigent individuals of Meriden and Connecticut to live with dignity in a secure homelike environment.

Full

A valid state nursing license is required for this position. Successful candidates will possess strong organizational and communication skills.

Responsibilities:

  • Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement
  • Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission
  • Facilitate problem-solving for complicated admissions
  • Ability to communicate with all members of the interdisciplinary teams.
  • Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
  • Updating diagnosis codes for billing
  • Complete and assure the accuracy of the MDS process for all residents
  • Updating Care Plans for resident changes in a timely fashion.
  • Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations

Qualifications for MDS Coordinator

  • 2 years experience in MDS-related experience
  • Current and valid state nursing license
  • Knowledge of state and federal regulations
  • Leadership skills
  • Decision-making skills
  • Interpersonal skills
  • Experience in a long-term care setting
