Job Description

Hiring Bonus for Full Time positions: $1,500 ($750 after working FT for 30 days AND $750 after working FT for 6 months).

Pay Range: $22 - $26/hr


It is important to provide a complete and relevant employment history. Pay is determined by verifiable experience.

Available Shifts

1) Fulltime: 6am-2pm, Mon- Friday

2) Fulltime: 2pm-10pm, Mon- Friday

3) Part-time: 3pm -7pm, Mon- Friday

4) PRN: As needed

Job Duties

The Medication Management LPN is responsible for providing medication management services to residents of Assisted Living. Orders, receives, and administers medications, processes physician's orders, and communicates with resident's family and physicians. Documents daily observations and services provided. Provides services in accordance with regulatory guidelines and compliance for Assisted Living Facilities. Additional duties include:

  • Reviews and follows up on daily communication logs.
  • Accurately documents daily observations and services provided and maintains accurate records.
  • Ensures that safety and infection control practices are continuously followed.
  • Works in conjunction with the Wellness Nurse to provide direct or indirect nursing care as needed or directed in accordance with state regulations and company policy.
  • Communicates with clients, families, and physicians as needed or as appropriate.
  • Observes physical or emotional changes in client's condition and reports them promptly.
  • Performs lab orders, prepares lab order forms and contacts laboratory provider to pick up lab specimens.

The ideal candidate has graduated from an accredited LPN program, holds a current unencumbered license as a Licensed Practical Nurse, and has one to two years' experience; preferably in the senior care field. Long-term care experience and experience with residents with dementia a plus. Current Health Care Provider CPR certification. Ability to represent the corporation in a professional manner and maintain a professional appearance; good human relation skills; strong verbal and written communication skills; proficient computer skills, strong organizational and documentation skills; ability to maintain confidentiality; highly motivated, energetic and compassionate. Must be reliable and possess a strong work ethic.
Physical Requirements: Ability to stand and walk for extended periods; ability to lift and assist senior adults with mobility.
The employee must have the ability to perform essential functions without posing a "direct threat" in the workplace.
