Job Description
The Intake Screener is a direct report to the Patient Intake Manager. This is a full-time, remote position.


Candidates must reside within driving distance to Milwaukee, WI for occasional on-site meetings.


  • Excellent time management skills
  • Excellent customer service, oral, and written communication skills
  • Serve as the first point of contact for new client referrals and schedule the intake appointment for intake associates and families
  • Monitor the intake voicemail and email inboxes for incoming referrals
  • Answer and transfer appropriate/necessary incoming calls
  • Ability to work under pressure and to be able to work effectively with a diverse workforce
  • Filing client reports, authorizations, and documents
  • Distribute incoming intake faxes to appropriate departments/personnel
  • Enter all referral and new client information into several databases including but not limited to Central Reach, M-Files, and Active Campaign
  • Routinely maintain all intake team tracking documents as directed
  • Follow all Standard Operating Procedures relating to the intake of new clients, as directed by team Lead, Manager, and Director
  • Excellent organizational skills
  • High School diploma
  • Remote position with occasional expectation to work from office setting
  • Proficient in Microsoft Office Suite and other software programs
  • Flexible; ability to adapt to change
  • Provide remote coverage for Intake Associates upon request
  • 1-2 years of experience in an administrative function
  • Obtain basic demographic information to complete the screening process
  • Strong listening/comprehension skills
  • Spanish bi-lingual preferred
  • Must possess superior interpersonal communication abilities and be a team player
