Intake Nurse Coordinator – Home Health
We are seeking a LPN for the position of Intake Nurse Coordinator. In this position, you will review and enter patient referral data received from inpatient facilities and community health providers. Through collaboration with referral sources and the Agency’s Business Development Team, you will facilitate the home health intake process. Responsibilities include insurance verification, obtaining service authorization, and triage and prioritization of patient referrals based on clinical need. You will also speak with patients and families to obtain pertinent information and address questions regarding VNA services. Position Summary:
The Intake Nurse Coordinator supports the Agency’s Intake process to facilitate the patient transition of care to home health services including approval for acceptance of patients, referral data entry, initial and subsequent visit authorizations, and assignment of new patient to the clinical team in a timely manner. Is present at beginning or end of department shift to ensure completion of referral processing.
Essential Functions:

• Communicates and follows up with business development staff/referral sources to assure completeness on individual patient referrals and educates referral sources on the VNA’s mission and full range of home health and community-based health services.
• Collects necessary demographic information, insurance information, and clinical history and provides clinical guidance to Intake staff to properly triage patients.
• Obtains initial visit authorization and receives periodic clinical reports to support subsequent visit authorization requests.

Required Minimum Skills:

• Must be flexible to manage changes in workflow volume and recognize urgency for completing tasks.
• Working knowledge of insurance reimbursement guidelines.
• Licensed Practical Nurse with current PA license.
• Excellent critical thinking skills with ability to solve unique problems as they arise and to discern when to consult the clinical manager.
• Knowledge of current home health admission criteria.
• 2 years nursing experience including at least one year in a home health or home care setting.
