Job Description
Position Summary


The Intake Admin is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Intake Admin will be responsible for triaging incoming calls to the intake department, managing all pending referrals and current hospitalized patients to facilitate timely transfer onto home health services, performing welcome calls for all pending admissions and provides weekend coverage for the intake department to accept and data enter referrals.


Position Specific Responsibilities


  • Coordinates with sales team on referral status.
  • Provides weekend coverage for the Intake department accepting and managing referrals via e-portals/referral centers, completing data entry of referrals, requesting additional documentation/referral documents as needed, and coordinating with sales team and agency leadership.
  • Triages incoming calls to the intake department.
  • Performs other duties as assigned
  • Assists with data entry of referral as needed
  • Updates inpatient events for pending admissions.
  • Manages all pending referrals and current hospitalized patients to identify patient discharge status and coordinate with scheduling department for timely Admission/ Resumption of care.
  • Performs welcome calls for all pending admission to confirm SOC date, payer source, contact info, address, primary care doctor, physician appointments, and ensure no duplication of services.


Education And/or Experience


  • Prior use of Electronic Medical Records (EMR) preferred.
  • Excellent verbal and written communication skills, detail oriented, organized, able to multitask.
  • High school diploma
  • General office procedure skills and knowledge of medical terminology
  • Minimum one year experience in a home care, hospice, personal care services or medical service setting, preferred.
  • Experience in dealing with a variety of customers in healthcare, including physicians, clinicians, and healthcare facilities.

  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Strong written and verbal communications.
  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.


Certifications, Licenses and Registrations


  • A valid driver’s license and auto liability insurance.
