Job Description
A rurally located hospital in southern Arizona is currently seeking a dedicated and experienced Nurse Coordinator to join their team at a Critical Access Hospital. The Clinical Care Coordinator will play a vital role in overseeing Case Management, Utilization Review, and Infection Preventionist functions within this small hospital. The Nurse Coordinator will have an important leadership role that works with the hospital and associated outpatient clinics. The hospital leadership are proud to operate in a team and mission-based setting.
The Registered Nurse Coordinator will be responsible for coordinating healthcare services and related activities for patients in various departments, including Acute, Observation, Swing, Hospice, and Emergency Department (ED). Collaborating with a multidisciplinary care team, facility case managers, and providers, the RN Care Coordinator will facilitate admission and discharge planning through team meetings. Additionally, the CAH Nurse Coordinator will work closely with insurance companies and case managers to ensure the necessary clinical information is provided as required and authorizations for admissions are obtained and documented in the patients' records.
Adherence to regulatory requirements set forth by Centers for Medicare Services (CMS) and Arizona Department of Health Services (ADHS) is paramount in this position. The Care Coordinator/UM/Infection Control Coordinator will enforce and ensure compliance with these regulations, as well as with hospital and departmental policies and procedures.
The Nurse Coordinator will actively participate in performance improvement and continuous quality improvement (CQI) activities. The Nurse Coordinator’s contributions to these initiatives will help enhance the overall quality of care provided to patients and promote a culture of excellence within thismedical facility.
Qualifications
  • Knowledge of Centers for Medicare Services (CMS) and Arizona Department of Health Services (ADHS) regulatory requirements.
  • Experience in Case Management, Utilization Review, and Infection Preventionist roles within a healthcare setting.
  • Commitment to continuous improvement and actively participating in performance improvement and continuous quality improvement (CQI) activities.
  • Familiarity with Critical Access Hospital (CAH) operations and regulations.
  • Current and unrestricted nursing license in the state of Arizona.
