As a Home Health Nurse, you will play a pivotal role in providing high-quality healthcare services to our patients in the comfort of their homes. Your primary responsibility will be to assess, plan, implement, and evaluate individualized care plans for patients with various medical conditions.

We are seeking a compassionate and dedicated Home Health Nurse (RN) to join our healthcare team. As a Home Health Nurse, you will play a pivotal role in providing high-quality healthcare services to our patients in the comfort of their homes. Your primary responsibility will be to assess, plan, implement, and evaluate individualized care plans for patients with various medical conditions.

- Intervisit mileage reimbursement $0.65/mile

- Schedule Details: M-F, 8 am – 5 pm; 40 hrs/ week.
- Location: King and Snohomish County
- Type: 13 weeks contract
- Administer medication, insulin, and IV/fluids, documenting thoroughly.
- Provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery.
- Assess patients for further injuries, infections, bedsores, etc., and deliver appropriate treatments.
- Assess and document patient conditions and report changes as needed (vitals, blood/stool/urine, glucose, etc.)
- Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.
- Inspect and care for wounds, change dressings, and assist with personal hygiene.
- Minimum of 18 months of experience as a Home Health Nurse (RN)
- Strong communication and interpersonal skills.
- Successfully complete and pass a background check and drug test.
- Two (2) positive work references
- High school diploma or equivalent
- Current BLS certification
- Oasis experience
