Description
The Home Health Aid provides personal care, home management and other related home health supportive services in accordance with the DOH Matrix entitled, "Permissible and Non-Permissible Activities: Home Health Aide Services", in order to assist the individual to continue living in their home environment when there are disruptions due to illness, disability, social disadvantage or other problems in the home. The Home Health Aide is under the direct supervision of the licensed nurse.
Essential Duties And Responsibilities
The Home Health Aid provides personal care, home management and other related home health supportive services in accordance with the DOH Matrix entitled, "Permissible and Non-Permissible Activities: Home Health Aide Services", in order to assist the individual to continue living in their home environment when there are disruptions due to illness, disability, social disadvantage or other problems in the home. The Home Health Aide is under the direct supervision of the licensed nurse.
Essential Duties And Responsibilities
- Keeps a detailed record of caregiving activities, including eating habits, mood changes, and physical activity levels
- Assists with activities of daily living, such as bathing, dressing, grooming, toileting, and ambulation
- Maintains a clean and orderly environment
- Provides companionship and engages in activities with the clients, such as reading, playing games, and going for walks
- Acts quickly and responsibly in cases of emergency
- Measures intake and output of fluids
- Observes appearance and gross behavioral changes in the client and reports to the agency supervisor.
- Perform other duties as assigned
- Assist clients with ambulation and mobility around the house or outside (i.e., doctor’s appointments, errands, etc.)
- Monitors vital signs and reports changes to the physician, nurse, or agency supervisor
- Collaborates with the client’s family members and other health care professionals to ensure the best possible outcome
- Adheres to all safety precautions and infection control procedures
- Plans, prepares, and serves meals in accordance with prescribed diet, and encourages fluid intake
- Assists with grocery shopping, picking up prescriptions, and attending doctor’s appointments
- Performs light housekeeping duties, such as laundry, vacuuming, dusting, and mopping
- Sympathetic attitude toward the care of the sick, and ability to deal effectively with the demands of the job
- High school or equivalent (preferred, not required)
- Good organization and communication skills and ability to read, write, understand, carry out directions, keep simple medical records, and function with minimal supervision.
- Ability to apply common sense understanding to carry out simple one or two step instructions.
- Ability to operate: walker, cane, crutches, wheelchair, commode, hospital bed, hoyer lift, household appliances (i.e., vacuum, refrigerator, stove, blender, toaster, etc.)
- Ability to deal with standardized situations with only occasional or no variables.
- Successful completion of a New York State Department of Health approved Home Health Aide training program as demonstrated by a valid Home Health Aide Certificate.
- Ability to portray exemplary customer service, speaking with respond, and avoid conflict.
- Authorized to work in the United States without sponsorship
