Job Description
Preps, scans and/or performs quality review on patient charts entered into the optical record system for permanent retention.


  • Pick up inpatient and same day charts from all nursing units and maintains accountability for charts from all nursing units within 48 hours of discharge.
  • Reconciles Scanning/Indexing worklists in Expanse.
  • Flexibility in scheduling may be necessary based on patient volume and patient care needs.
  • Must be able to identify scanner, printer and fax equipment problems and seek immediate resolution.
  • Maintain office area to include stocking supplies, cleaning, keeping the area organized, making appropriate person aware of needed supplies and/or services, and working with others to ensure shift flow.
  • Required skills must include a thorough working knowledge of chart content and chart order to enable assembly of a quality product of all patient type records.
  • Performs Indexing on all charts and transfer data to permanent medical record.
  • Accurately prepares charts for scanning. Responsible for document entry via all scanning equipment and uploading from shared folders via the shared drive.
  • Release patient information to nursing units and other healthcare facilities as appropriate for continuation of care.

  • Must be able to read, speak and write English.
