“The Fetal Cardiology Program, which includes specialists from the Heart Center and The Chicago Institute for Fetal Health, ensures that families expecting a new baby with heart concerns receive the best diagnostic and counseling care available. Our fetal heart program provides specialized ultrasound imaging of the fetal heart during pregnancy (fetal echocardiography). Each year, our pediatric cardiologists perform more than 2,000 fetal echocardiograms at multiple sites in the greater Chicago area, with a diagnostic accuracy that exceeds 95%. The Fetal RN Coordinator works closely with our families, Chicago Institute for Fetal Health, CICU and Fetal Cardiologists to determine appropriate care plans and further testing, as well as follows the patients throughout pregnancy and post-birth. This position will be joining our 3 established Fetal RN Coordinators as we continue to grow our program at Lurie Children's.”


Job Duties:


  • High level of initiative and autonomy.
  • Previous successful experience in new program development, coordination, and marketing.
  • Provides expert care coordination, education, consultation and advocacy.
  • Coordinates a patient hand-off to the Lurie Children’s Coordinated Care Entity when appropriate.
  • Transitions the patient from the inpatient to the outpatient setting. Supporting patient education for the clinical diagnosis as well as educating families on community resources which are available.
  • The ability to effectively communicate with patients/families, physicians, hospital staff, social agencies, school personnel and community resources.
  • Maintains a clinical database to develop and track key performance indicators.
  • Works with physicians, surgeons, consulting services, and hospital personnel to develop an organized care plan for patients who have been identified to participate in the program.
  • Current licensure as a registered professional nurse in the State of Illinois is required.
  • A BSN degree from an accredited School of Nursing is required. A master’s degree from an accredited school of nursing is preferred.
  • Documents all team, physician and patient/family communication and concerns pertaining to coordination of care and services in the medical record.
  • Minimum of 3 years relevant clinical experience, preferably in case management.
  • Acts as the primary resource for care coordination for families. Completes assessments, identifies goals and objectives for performance improvements in order to provide the best patient care, optimum customer service and improve overall satisfaction.
  • Excellent organizational skills, including ability to effectively manage multiple competing responsibilities.
  • Coordinates the specific program activities for the patient population.
  • Works collaboratively with the clinical case manager, social work, outside agencies and families to coordinate care of the complex patient.
  • Proficient in a variety of computer programs, including Word, Excel, and PowerPoint and working knowledge of EPIC.
  • Other job functions as assigned. Minimum Qualifications:
  • Communicates essential information to patients, families, and members of the care team.
  • Performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others.
