Job Description
Pulaski Health & Rehab Center in Pulaski, VA is seeking a Part-Time Discharge Planning Assistant. Pulaski Health & Rehab provides a high volume of short term nursing and rehabilitation services. We are seeking a candidate that can provide discharge planning as well as comprehensive medically related social services to our patients. Responsibilities include MDS, Care Plans, Assessments and timely documentation operating within the framework for the company's standards for best practice in accordance with all state and federal regulations.


The qualified candidates will have a 4-year degree in Social Work, Psychology, Sociology, Counseling, Criminal Justice, or other human services related degree.

Primary responsibilities include but are not limited to the following


  • Maintains a clean, sanitary environment, minimizing any infection risk.
  • Promotes and enhances the safety of patients, families, and staff.
  • Cleans and services patient rooms, baths, offices, dining rooms, and other ancillary areas.


Job qualifications include but are not limited to


  • Ability to communicate, comprehend, understand and write messages, and follow directions.
  • Prior housekeeping experience preferred.
  • Ability to read and write.
  • Prior health care experience preferred.
  • Ability to effectively follow simple oral and written instructions.
  • Ability to operate equipment.
