Discharge Coordinator

General Summary


Under limited supervision, the discharge coordinator is responsible for assisting in the implementation of the discharge and aftercare plans on the inpatient psychiatric unit. The coordinator is a member of the care team and support team and has accountability to function as a part of the team to accomplish patient outcomes identified in the pathway/ plan of care.


Principal Responsibilities and Tasks


The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified.


  • Demonstrates awareness and sensitivity to patient/ family rights, as identifies within the institution.
  • Participates as a member of the Patient Care Support Team to assist in the implementation of the discharge plan/after care plan/transfer/admission, including collaboration with Care/ Support Team members to meet patient’s needs
  • Participates in treatment meetings/shift report.
  • Ensures the confidentiality and respects the rights of patients and employees and the confidentiality of all hospital and departmental documents.
  • Applies the principles of growth and development over the life span and assesses data reflective of the patient’s requirements relative to his/ her age specific needs.
  • Gathers pertinent medical and social data regarding patients, through consultation with psychiatrist, social worker, MHC and/or patient care team members.
  • Assists in the development of an appropriate discharge plan, transfer or admission for the patient.


Qualifications


Education and Experience


  • One (1) year of experience in medical/ psychiatric setting required.
  • Knowledge of Microsoft Windows or successful completion of a hospital training program required. Comprehensive knowledge of patient medical records, health insurance provisions, psychiatric diagnoses, and utilization review practices.
  • High school diploma or equivalent required. Associate degree preferred.


Working Conditions


Works inside a clean. Well-lighted environment. May be exposed to multiple crisis situations. Time will be spent in office environment and in patient care areas. May be exposed to verbal abuse by patients. Expected to work overtime to complete a tasks that cannot wait until the next working day. Potential for minimal exposure to communicable diseases and hazardous materials, requiring the observance of Standard (Universal) precautions and safe handling practices.


Physical Requirements


  • Near visual acuity is necessary for accurate reading. Talking and hearing are necessary for telephone conversations and consulting patients/ families, communicating with staff members and visitors.
  • Standing, bending, ascending/ descending stairs, reaching, lifting, and pushing/ pulling is required 0-3 hours/ shifts.
  • Working, sitting, carrying is required 3-5 hours/ shift. Weight handled is less than 10 pounds.
