Job Summary:


We are seeking a full-time Director of Clinical Services to lead our Clinical Team. This talented and skilled individual will be responsible for clinically assessing clients and their home environments, developing each individualized client care plan, and providing and coordinating necessary for clinical services to provide exceptional care for each client. The Director of Clinical Services acts as the administrator, with a focus on quality assurance & quality control and on learning & development.


This clinical and administrative position is primarily based out of our Stamford, CT headquarters and requires travel to clients and prospects within close territories to ensure the overall implementation, delivery, and coordination of our services. Responsibilities:


  • Maintain and further develop in-house clinical education and training materials for caregivers through oversight of the Learning & Development Manager
  • Develop and maintain client care plans, working in partnership with other healthcare professionals on the client’s clinical team (e.g. physicians, physical therapists, occupational therapists, discharge planners, social workers, and other healthcare professionals as needed)
  • Overall administration and development of necessary policies and procedures for the provision of clinical support to clients and caregivers
  • Other related duties and responsibilities as assigned.
  • Communicate and coordinate with client’s outside clinical team
  • Identify individual client needs in order to provide high-quality care and perform initial and periodic onsite client assessments.
  • Lead the Quality Assurance & Quality Control Team and the Learning & Development Department.
  • Support and coordinate clinical care for clients
  • Oversee development and coordination of part time and per diem RNs on staff
  • Support and advise caregivers, clients and their families on clinical issues
  • Ensure quality control and assurance of direct patient care
  • Supervise the Learning & Development Manager and oversee the training program to ensure proper instruction to caregivers regarding each client's needs and care plan


Requirements

  • Familiarity and working knowledge of advanced directives
  • Commensurate technical skills and ability to multi-task.
  • Strong interpersonal skills for developing relationships with clients, their families and our caregivers.
  • Strong administrative, coordination and communication skills.
  • Extensive hands-on clinical experience.
  • Ability to work with a variety of health care professionals, clients and caregivers.


Preferred:


  • 5+ years of experience as a Clinical Manager, Coordinator of Care or RN Supervisor with a home care or home healthcare company or in an institutional setting (i.e. assisted living, skilled nursing facility, hospital).
  • Experience working with older adults and their associated comorbidities.


Qualifications:


  • Bachelor's degree in Nursing, Social Work, or a related field
  • Excellent verbal, written, and communication skills
  • Must be able to work flexible hours, including some evening and weekend hours.
  • Ability to work onsite in the corporate office in Stamford, CT, as well as ability to travel throughout our area of service.
  • Experience working with older adults and their associated comorbidities.
  • Registered Nurse (“RN”) licensed to practice in New York and/or Connecticut as specified by each state’s regulations; willingness to become licensed in New Jersey
  • Knowledge of age-related illnesses, including understanding of Palliative & Hospice Care.
  • COVID-19 vaccinated.
  • 5+ years of experience as a Clinical Manager, Coordinator of Care or RN Supervisor with a home care or home healthcare company or in an institutional setting (i.e. assisted living, skilled nursing facility, hospital). Minimum 2+ years of experience as an RN
  • Experience in direct patient care and assessments/observation of clinical conditions; with strong clinical skills
