Job Description
MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.


Program Assistant Adult Day Would you like to work in a rewarding environment with fantastic co-workers? This position is for you! You can make a difference by providing care to the individuals we serve in our community-based Day Program.


  • Assist with the development and implementation of individual and group activity programs, and responsible for the care, safety, security, and social/recreation activities for the individuals receiving support.
  • Provide assistance in the daily care of health impaired older adults or individuals in our care with brain injuries, intellectual/ developmental disabilities, emotional or behavioral disability or mental diagnosis.
  • Assist individuals receiving care with daily living, including personal hygiene, bathing, bathroom assistance, grooming, preparing meals, and medication administration.
  • Transport to medical appointments, errands, shopping or activities in the community.
  • Provide guidance with social skills, advocacy and self-esteem and teach strategies to live as independently as possible.


Qualifications:
  • Valid driver’s license, access to reliable vehicle with current registration, and insurance
  • Strong attention to detail and effective communication skills to manage relationships
  • High school diploma or its equivalent and must be 18 years of age
  • Minimum one year experience in providing services to individuals with intellectual and developmental disabilities, preferably with older adults
  • Proficient in vital signs, weights, Activities of Daily Living (ADLs) and independent ADLs
  • A reliable, responsible attitude and a compassionate approach
  • Competence in CPR and First Aid
  • Physical in the past 12 months and annual TB test
