Job Description
Hours: 15 hours per week


Hiring Range: $ 18.00 - $20.00/hour


Funding Period through 8/1/2024


CHA requires employees to be up to date on all vaccines, including flu and COVID vaccines, and provides vaccination for staff. Vaccines must be up to date, or you must be willing to update them upon hire.


The community Health Worker (CHW) is a patient advocate or liaison in a community setting. Building trust and promoting encouragement are two key objectives. The CHW helps patients access community services specific to their needs. The CHW assists in creating community outreach programs and activities. These outreach programs are designed to promote, maintain and improve the health of the community.


Primary Responsibilities


  • Act as advocate for individuals and communities within the health and social service systems
  • Present COVID-19 vaccine information among priority populations
  • Provide health information to community members and encourage healthy living habits
  • Conduct intensive outreach efforts to community members and agencies serving vulnerable populations
  • Assist patients in referrals, follow-ups and completing applications for services


Education/Experience: High School or equivalent and 1-2 years of related experience. Written and oral fluency in Spanish and English preferred.


Skills/Abilities/Knowledge


  • Proficient in medical and behavioral terminology
  • Written and oral fluency of both English and Spanish is preferred
  • Must have excellent communication skills with patients, staff, providers and community members
  • Possess conflict management and resolution skills
  • Knowledge of electronic medical records system is preferred
