Community Clinic of Maui, Inc. dba Malama I Ke Ola Health Center is a Federally Qualified Health Center (FQHC) and a fully integrated behavioral health, dental, and primary care system. Our 100+ professionals and providers serve the people of Maui at three clinics—two locations in Wailuku and one in Lahaina. We have behavioral health providers who partner with the primary care clinicians to assist in a vast number of activities including warm handoffs for acute behavioral health crisis management, substance abuse counseling, tobacco cessation counseling, chronic disease self management counseling, and long-term individual and family therapy. We have been serving the Maui community since 1993. Since 2015, we have served over 11,000 patients of which 97% are living at or below 200% of the Federal poverty level. FULL TIME (40 hours per week) with the ability to work evenings and/or weekends :
The Community Health Worker: contributes to meeting the comprehensive needs of the patient from the social determinants of health to direct medical services. Under the Primary Care Medical Home (PCMH) model, the Community Health Worker works in conjunction with the clinical team in the specific populations to place services and contribute to patient outreach, education, navigation, social support, medical follow-up needs, and advocacy, to achieve positive outcomes for chronic disease management and risk factor reduction.

REQUIREMENTS
  • Provides, delivers, and/or manages culturally and age-appropriate services such as:
  • Identifies resources available for patients
  • Care management plans
  • Education and communication materials, educational tools, multimodality approaches to care in multiple languages
  • Collaborates with adult medicine provider team to provide patient advocacy, to include working with patients, partners, families and caregivers
  • Eligibility referrals
  • Outreach to health center patients and non-patients which may include unsheltered individuals and those with substance use.
  • Entry Screening of all patients and visitors to our Ooka health center to clear for flu/flu-like symptoms or arrange an escort to a respiratory room.
  • Coordination of transport
  • Assistance with appointment scheduling
  • Psychosocial support
  • Provides culturally and age-appropriate education to include instructional materials, with multiple language options, that address chronic disease management
  • Patient Assistance Programs (PAP) to provide financial assistance for accessing alternative care food vouchers for access to healthy foods at the HC’s farmers market.
  • Written and oral fluency in English required.: Additional fluency preferred yet not required: Ilocano, Tagalog, Native Hawaiian, Marshallese, Chuukese, Pompeian, Kosraean, Yapese, Chamorro, and/or Palauan.:
  • Assists with patient goal setting and self-monitoring
  • Implements and performs outreach services that include contacting assigned patients who have yet to establish care, educating patients about the range of services available to them at the HC, and expansion of translation and/or interpretation services available to patients.
  • Translation and/or interpretation services
  • Negative TB clearance within the past 12 months.
  • BLS 1st aid/CPR
  • Must be fully vaccinated against COVID-19
  • Valid driver license and access to an insured vehicle.

QUALIFICATIONS
  • Experience working in a community-based setting for at least 1 to 2 years preferred
  • Good communication skills, such as listening well, and using language appropriately
  • Ability to initiate and maintain positive working relationships with staff and other organizations
  • - Understand the community served - community connectedness
  • Knowledge of some medical terminology preferred
  • Ability and willingness to provide emotional support, encouragement and motivation to patients.
  • Basic computer skills
  • Experience working in the multi-cultural setting unique to Maui
  • All clinic employees are expected to show a willingness to learn and demonstrate all aspects of patient/client care, the principles of trauma-informed care and practice.
  • High School Diploma or equivalent.
