Job Description
The Clinical Pharmacy Specialist in Infectious Diseases and Antimicrobial Stewardship is responsible for the provision of safe, effective, appropriate and cost-effective medication therapy to patients in the area of Infectious Disease and oversees the hospital’s antibiotic stewardship program. He/she works collaboratively with antimicrobial stewardship physician leader to provide specialized pharmaceutical care, review and report on antimicrobial stewardship data for the hospital, develop and apply medication use policies, participate in staff member competency, and participate in research activities. He/she serves as a key leader, along with the ASP physician leader, to ensure compliance with The Joint Commission, CMS and all regulatory standards for antimicrobial stewardship.
Required Skills
  • Provides ASP performance updates and education to hospital leaders, physicians, nurses, pharmacists, and other health care providers
  • Assists in developing and maintaining the anti-infective formulary, anti-infective guidelines and use evaluations, infectious disease related clinical pathways, and order sets
  • Supports the pharmacists with addressing clinical questions as related to ID and ASP to enhance patient care
  • Assists in leading the Antimicrobial Stewardship Committee, a multi-disciplinary antimicrobial stewardship dedicated to promoting excellence in antibiotic utilization, by developing the agenda, running the meetings and minute-taking
  • Provides expertise on appropriate antimicrobial therapy, antibiotic de-escalation, and dose optimization.
  • Assists the pharmacists in the clinical pharmacy services, including but not limited to, pharmacokinetics consults, renal dosing, neonatal/pediatric/geriatric dosing management, route of optimization for antibiotics
  • Ensures compliance to The Joint Commission, CMS and all regulatory standards through active leadership of the Antimicrobial Stewardship Committee
  • Serves as a preceptor for pharmacy students as necessary
  • Develops performance indicators and dashboards for tracking antimicrobial utilization and other Antimicrobial Stewardship Program (ASP) initiatives as well as report on other applicable ASP metrics to be compliant with regulatory standards and identifying opportunities to optimize antimicrobial use and patient care
  • Participates in ASP rounds and antibiotic utilization reviews with Infectious Disease physicians
  • Acts as the co-leader of the antimicrobial stewardship program (ASP) along with the ID ASP physician to design, implement, communicate, and document pharmacy-based and hospital-wide performance improvement initiatives to ensure appropriate utilization of antimicrobials (e.g., order set and clinical pathway development, antibiotic time-out processes, automatic IV to PO and antibiotic dose adjustments by pharmacy, antibiotic review at transitions of care)
  • Serves as a liaison between the Department of Pharmacy and Division of Infectious Diseases
  • Reviews literature and publications to provide recommendations for improvement to the antimicrobial medication use process
  • Conducts daily ASP activities including prospective audit of antimicrobial use, review of all positive blood culture results, respiratory viral panel results and provide expertise on appropriate antimicrobial therapy, antibiotic de-escalation, and dose optimization.
Qualifications/Requirements
Required Experience
 Current Connecticut Pharmacist License
 Graduate from an accredited College of Pharmacy required
 Advanced degree (PharmD or MS) or equivalent clinical practice experience; PharmD degree and/or ASHP accredited residency
 Board Certification preferred
