As a Registered Nurse Clinical Manager, here’s a big-picture view of what you’ll do:

• Ongoing interdisciplinary assessment of the patient.
• Development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Effectively communicates with other members of the interdisciplinary healthcare team.
• Ensures timely and accurate clinical notes including addressing patient’s progress towards patient goals.
• Ensure services that are ordered by the physician are provided as indicated in the plan of care.
• Coordinates services including patient care and referrals and assures assignments.

Requirements A few must-haves for an RN Clinical Manager:

• Active license in the Virginia and in good standing
• A licensed registered nurse.
• Graduate of an accredited school for applicable discipline.
• Three (3) years home care experience preferred.
• Willing to travel (up to 25%)
• Able to demonstrate strong organizational and managerial skills.
• Two (2) years management experience preferred.
