Job Summary


The Home Health Clinical Manager reports to CareAparent's SVP of Home Health Operations and has general management responsibility for services provided by the Home Health program, with direct supervisory responsibility for the program's Clinical staff and Team Assistant(s). This position serves as a pivotal point in communication with field staff, support staff, referral sources and other managers/departments to ensure extraordinary service delivery. This position reviews, plans and evaluates the care of patients in accordance with organizational policies and evidence-based standards of professional clinical practice. This position provides orientation, training, and support for both newly hired and existing clinicians. This position serves as a mentor for case management and participates in the assessment and planning of appropriate care.


Essential Duties And Responsibilities


  • Participates in continuing education to continually improve staff skills and abilities; leads or participates in training and annual training (s) as directed by supervisor or agency.
  • Prepares and manages all documentation and reporting requirements including but not limited to QA reports, performance evaluations, productivity, time sheets, and schedules and workflow.
  • Oversees intake, scheduling, and authorization processes to ensure a smooth workflow of operations.
  • Ensures effective staff utilization to meet staffing and workload requirements and support the delivery of quality patient care and services.
  • Manage clinical staff by assigning and delegating tasks as needed.
  • Coordinate and lead regular team meetings with clinical staff.
  • Monitors policies, procedures, and practice to assures compliance with regulations related to certification, accreditation, and organization in the care delivery process
  • Provide direct supervision (approximately 50% direct observation in field, 50% office) of field performance to competence in KPBs, technical assistance and support of Home Health staff.
  • Assist with performing patient visits when the need arises to ensure appropriate coverage.
  • Coach, mentor, and train/develop staff, oversee new employee on-boarding and orientation, performance management, evaluation and disciplinary action if needed.
  • Develops staffing plans; works with human resources staff to recruit and hire staff; assists with coordinating comprehensive orientation to, but not limited to, new staff in area of responsibility.
  • Facilitate patient and employee scheduling/assignment in collaboration with Clinical Coordinator, Intake Coordinator, and administrative support to ensure timely initiation of care.
  • Serves on the QAPI Committee. Provide necessary follow-up/training with staff to meet Key Performance Behaviors (KPB). Implement care delivery best practices into the field consistent with outcomes of QAPI initiatives. Monitor care delivery in the field for competence and compliance.
  • Promotes safe work practices and work environments to protect the safety and wellbeing of employees and patients.
  • Manage staff to ensure clinicians meet productivity expectations.
  • Monitors and manages quality improvement and assurance initiatives for clinical operations, including but not limited to staffing schedules, field staff, and patient flow, auditing for areas of responsibility
  • In the absence of the Director of Nursing, oversees all home health clinical operations.
  • Establishes accountabilities and performance goals and provides counseling, coaching, feedback, recognition, training, and development to staff.

Minimum Qualifications


  • American Heart Association BLS/CPR.
  • Two years of experience in Homecare
  • Valid Registered Nurse license in the state of Minnesota.
  • Excellent analytical and interpersonal skills required. Certification preferred.
  • BSN preferred
  • One year of supervisory experience is required.
  • Valid driver's license with an insured, reliable vehicle.
