Requires daily travel within Hardin market. Under direction from the Director of Clinical Outcomes for Value Based Care, responsible for implementation of education to Clinical Integration Network and Accountable Care Organization members and their support staff regarding CIN, payer partnerships and ACO program measure specifications. Subject matter expert on various measure sources including CMS and Commercial payers. Researches and maintains database of best practices for use in the CIN. Organizes and presents accurate reports to instill confidence in both programs. Maintains a positive working relationship and ongoing communication with CIN, payer partnerships and ACO staff, physicians and their support staff, hospital staff members, physician leadership and hospital leadership. Assists with educational effort regarding the CIN, payer partnerships and ACO programs, IT implementation and maintenance, and support services. Assists with ACO and payer audit annually and works closely with ACO, payers and Baptist Health System staff seamlessly. Proactively identifies and reports any special needs and potential problems to leadership to ensure the programs’ success. Demonstrates necessary interpersonal skills and maintains a positive and professional atmosphere in the department. Follows system and Departmental Policies and Procedures.

  • Minimum of five years recent clinical experience in outpatient/inpatient setting. Understanding of quality outcomes preferred.
  • Should excel in both written and verbal communication and have a strong financial aptitude.
  • Minimum education of Licensed Practical Nurse or Certified Medical Assistant or higher.
  • Knowledge of electronic medical records (EMR) and adaptability to learn new EMR’s platforms as needed.
  • Proficient with Excel, Access, Word, PowerPoint and other means of presentation of materials required.
  • A proven ability to work closely and collaboratively with healthcare professionals and leadership as an essential component of their experience.
