Job purpose


The primary purpose of the position is to help people with substance use disorders enter into a continuum of care.


This position is responsible for providing chemical health assessments, making appropriate referrals, and securing funding when necessary.


Duties and responsibilities


The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.


  • Conduct chemical health assessment interviews.
  • Maintain professional license.
  • Attend all in-service trainings.
  • Enter the proper information in the Meridian Behavioral Health Electronic Medical Record (Procentive), so that the assessment can be billed.
  • Schedule assessment appointments in the Meridian Behavioral Health Electronic Medical Record (Procentive).
  • Communicate effectively with members of the intake team and other clinic personnel.
  • Fill out the proper paperwork that accompanies the chemical health assessment, i.e., releases, application for services, Client Placement Agreement, funding requests for Residential or Outpatient services, notification paperwork, etc.
  • Attend R25 Assessor meetings on a monthly basis
  • Establish and maintain professional relationships with funding sources and potential referral sites.

Qualifications Required To Perform Essential Job Functions


  • CPR and first aid certified.
  • 2-3 years related experience required or any combination of education and experience that provides the required knowledge, skills, and abilities.
  • Must document 12 hours of training in co-occurring mental health problems and chemical abuse dependency that includes competencies related to: philosophy, screening, assessment, diagnosis, treatment planning, documentation, programming, medication, collaboration, mental health consultation, and discharge planning.
  • Must have current Minnesota State Licensure as an Alcohol and Drug Counselor or equivalent.
  • A minimum of two years free from chemical abuse problems.


Must know and understand all rules, regulations, and requirements of the assigned program.


Required Skills, Knowledge And Ability


  • Competent critical thinking skills in order to make appropriate recommendations and referrals.
  • Ability to communicate with a variety of people from diverse cultures, socioeconomic, and educational backgrounds.
  • Knowledge of Minnesota Matrix: Rule 25 Risk Descriptions Guide and Rule 25 Treatment Planning Decision Guide.
  • Knowledge of the Meridian Behavioral Health Electronic Medical Record (Procentive).
  • Computer skills
  • Knowledge of what the various funding sources require in order to secure the funding for services.
  • Strong verbal skills and ability to communicate abstract concepts in a simple format.


Physical Requirement


  • Occasionally reaching for and lifting client charts, 5 lbs, 10% of time
  • Heavy Data Entry on computer, 25% of time
  • Extended periods of sitting, 80% of time
  • Frequent handling of paperwork, binders, writing
