Pay Range: $79,100 - $119,646 USD Annually


Purpose of Job:

Improve the overall health of the communities we serve by supporting patients with complex chronic conditions as follows:

Essential Duties and Responsibilities:

  • Supports patient care teams in creating care plans and helps to implement them with the goal of better patient engagement with care teams and adherence to prescribed therapies. Performs patient education as needed.
  • Coordinates successful transitions of care for patients discharging from hospital settings.
  • Collaborates with case management and other resources to assist with social determinants of health needs and barriers.
  • Performs thorough patient chart reviews and conducts patient interviews to identify clinical and social determinants of health barriers.
  • Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.
  • Analyzes available registries and reports to identify patients with complex chronic conditions in need of additional support, for the purpose of implementing appropriate care plans.
  • Assists with tracking and presenting outcomes for assigned populations to leadership.
  • Works collaboratively with patients, CHAS Health care teams, and outside entities to coordinate care.

Qualifications:

Education: Bachelor's degree in nursing from an accredited school of nursing or equivalent experience required. Registered Nurse (RN) license in the State of Washington required upon hire and eligible to obtain RN license in Idaho. Community health setting experience preferred. Experience with Chronic Disease Management preferred. Experience with electronic health records preferred.

Skills: Excellent customer service, oral and written communications skills required. Compassion for patients required. Must be able to work well in a team environment. Ability to multi-task in fast-paced work environment, with ability to anticipate future needs required. Commitment to supporting a safe, respectful, equitable, and inclusive environment required. Computer skills required. CPR/AED certified for a Health Care Provider required. Precepts students as needed. Valid drivers’ license and insurance required.

Physical Demands:

While performing the duties of this job, the employee is required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one-third and two-thirds of the day. Communicating by talking/hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
