Primary Purpose


In partnership with the greater Fairfield County Coordinated Access Network (CAN), New Reach provides supportive housing and rapid-rehousing case management services to assist individuals and families with exiting homelessness into permanent housing through the provision of time-limited housing supports and strategies with the ultimate goal of housing stability.


Essential Functions


  • Work collaboratively and maintain positive, professional communication with property owners/managers, subsidy administrators, and community providers to support housing acquisition, retention, and overall stability
  • Refer and support engagement in appropriate services to meet household needs
  • Assist clients with preparing for housing, including affordable housing search, transition to housing, tenancy skills building, fiscal preparedness for independent rent payment, and housing maintenance
  • Complete coordinated screenings and intakes with the Coordinated Access Network (CAN).
  • Assist clients with identifying unaddressed needs related to benefits, employment and income, education, housing, medical conditions, mental health, substance use and recovery, family and/or community-based supports, and access to mainstream services.
  • Actively participate in agency and community-based meetings and trainings to engage in programmatic, operational, and client service best practices
  • Maintain timely and accurate documentation for all client files, including assessments, service plans, client services, referral outcomes, housing inspections, rental assistance, and case notes. Comply with agency, funder, and statewide HUD HMIS standards.
  • Provide comprehensive, time-limited, home and community-based case management services.
  • Assist in program development and quality assurance activities as directed.
  • Report to work at 850 Park Ave. Bridgeport, CT during the regular business hours of 8:30am to 5:00pm, with flexibility to work occasional evenings as needed to perform client facing services.
  • Develop a strong community presence to educate others about the program, advocate for housing rights, and mediate housing concerns effectively


Qualifications


  • High level of organizational skills to prioritize and manage multiple projects simultaneously.
  • BA/BS in Human Services or related field, OR Associates Degree and 2 years of human services experience required.
  • Highly motivated, with excellent interpersonal, oral, and written communication skills.
  • Ability to work independently and as part of a team, to support the mission of New Reach.
  • Valid driver's license and access to reliable transportation with proof of insurance.
  • Proficiency in Microsoft Outlook, Word, Excel, and Power Point.
  • Ability to provide intensive and comprehensive services to households experiencing homelessness, including advocacy and mediation skills.


Preferred Qualifications


  • Experience with housing, housing law, and provision of services to the identified population preferred.
  • Bilingual/Bicultural preferred.

Physical Demands


  • While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The associate must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by the job include close vision, distance vision, and the ability to adjust focus.
