Job Description
Case Manager - Young Adult Services


Must be able to work one of the following schedules:


  • Sunday 8-4, Monday & Tuesday 7-3, Wednesday & Thursday 9-5
  • Monday - Thursday 9-5, Friday 3-11


$21.07 - $27.08 per hour


Position Summary


Improves the living experience and recovery process of clients in Young Adult Services through support, advocacy, teaching and concrete assistance


Duties And Responsibilities


  • Provide activities of daily living supervision, assistance and training promoting independent functioning in consumers.
  • Facilitate therapeutic/recreational/educational groups.
  • Ensure entitlements and appropriate services for consumers.
  • Provide transportation for consumers.
  • Insure consumers attend scheduled appointments/activities.
  • Maintain consumer records.
  • Perform any other departmental or agency related duties or special projects as directed by supervisor.


Qualifications


Bachelor's degree required, with major in human services preferred.


Ability to work a flexible schedule.


Knowledge of behavioral health issues.


Good communication and documentation skills.


Computer literacy.


Ability to prioritize and organize schedules/demands.


CPR/First Aid certified or willing to become certified.


Valid driver's license and access to reliable transportation.
