Role Description
Recording and monitoring a patient's vital signs. Promoting well-being by educating patients on self-care techniques. Consulting with doctors and other medical staff to determine the optimal treatment plan. Administering over-the-counter and non-intravenous medications
Registered Nurse Responsibilities
Conducting intake assessments with patients and families.
Ensuring that families feel sufficiently equipped and supported.
Developing individualized treatment plans that account for physical, financial, and other constraints.
Adjusting treatment protocols, as needed.
Suggesting and coordinating appropriate follow-up services to promote wellness.
Ensuring that patients have transport to and from their homes and all pertinent healthcare facilities.
Meeting with members of healthcare teams to discuss patients' progress, and to deliberate on appropriate care.
Checking to see whether patients have sufficient insurance coverage or finances to pay for their treatment.
Devising innovative solutions or suggesting appropriate referrals for patients who cannot afford to access treatment.
Registered Nurse Requirements
Bachelor degree or master degree in nursing.
Pass the National Council Licensure Examination or NCLEX-RN.
Current state licensure as a registered nurse and BLS certification required.
American Heart Association BLS/CPR
ACLS certification
Ability to effectively communicate with patients, families, physicians and hospital staff.
Basic computer skills.
Professional, friendly attitude.
Experience in home health care.
Experience as RN Case Manger.
Document Requirements
Active RN license required, Must have recent CM experience and CM certification preferred, BLS("American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS. Must include an image the front of card as well as an image of the back of the card.")
