Overview

We are seeking a highly motivated, innovative, enthusiastic and inclusive staff member to join our Aging and Adult Services Division as a Case Manager!

Options for Long Term Care (OLTC) provides access to publicly funded programs that support people with disabling functional conditions and limited finances. These programs are designed to help people continue to live in their own homes or in other community settings, as an alternative to nursing home care. OLTC currently provides services to approximately 1300 Larimer County residents outside of nursing facilities!

Responsibilities

Specifically, this position:

  • Manages a caseload of approximately 100 + individuals on an HCBS waiver and/or completes initial assessments for individuals applying for Long Term Care Medicaid programs.
  • Provides crisis intervention as needed to assist individuals in decision making and problem resolution; acts as advocate for the individual.
  • Educates individuals regarding Medicaid benefits, programs and non-Medicaid services in the community. Uses Medicaid in the most cost-effective manner.
  • Monitors individuals quarterly or more often to ensure the ongoing provision of need and satisfaction of services.
  • Coordinates services including scope and duration while ensuring timeliness of service delivery.
  • Has the ability to create a service allocation based upon functional need.
  • Assesses for nursing facility placement when appropriate.
  • Works in various settings including but not limited to: open office work environment, individual’s home and in the community.
  • Performs assessments and ongoing case management for older and disabled low-income individuals needing long term care services in their own homes or alternative care facilities.
  • Documents all contacts and assessments in multiple county and state systems.

We are looking for a candidate that will:

  • Have knowledge of Long Term Care Medicaid Programs, eligibility requirements for HCBS waivers, functional eligibility requirements, and service authorization.

  • Have excellent time management and organization skills.

  • Take initiative and work efficiently.
  • Use problem solving skills and critical thinking skills.
  • Be able to shift from planned calendar events to handle unplanned situations.

Qualifications

_All candidates will be screened for the following required minimum qualifications. Any candidate who does not meet all of the qualifications listed as required will be eliminated from the process.

Depending on qualifications, this position will hire as either Case Manager I or II.

Case Manager I

  • Equivalent to a Bachelor's Degree from an accredited college or university required. Combination of education and/or experience may be substituted on a year for year basis.
  • Possession of a valid Driver's License by date of hire required.
  • Complete training and/or certification as mandated by the assigned program required.

Case Manager II

  • One (1) year of experience in a directly related field or in the performance of similar duties and responsibilities required.
  • Possession of a valid Driver's License by date of hire required.
  • Complete training and/or certification as mandated by the assigned program required.
  • Equivalent to a Bachelor's Degree from an accredited college or university required. Combination of education and/or experience may be substituted on a year for year basis.

The typical schedule for this position is Monday through Friday from 8am to 5pm.

This is a hybrid position and will require at least one day/week in the office. You will be required to have adequate internet access and space for remote work. Larimer County provides an annual remote work stipend to assist with equipment and office supply needs for remote workspaces.

While working in the office, this position will be located at one of the following three locations: 1501 Blue Spruce Drive, Fort Collins, CO 2555 Midpoint Drive, Fort Collins, CO 200 Peridot Avenue, Loveland, CO

This position requires a vaccine compliance badge.

All new employees and rehires will be required to submit fingerprints for a CBI criminal background check (c.r.s. 26-6-104).

All staff working in the Options for Long Term Care Unit, Adult Protection Unit, or Office on Aging will be required to submit information for a Colorado Adult Protective Services data system check (c.r.s. 26-3-1-111).

An offer of employment is contingent upon the successful completion of a pre-employment drug screen. As marijuana is an illegal substance under federal law, testing positive for marijuana (medical or otherwise) or any other substance for which the county tests, will result in the contingent offer of employment being revoked. A successful completion of the drug screen is defined as obtaining a negative result.
