Job Description
Position Summary
This Care Site Coordinator position functions as the support for the CHRIS Counseling Center for all consumers and families.
Role And Responsibilities
  • The primary duties include but are not limited to the following:
Provide positive role modeling to consumers/families.
  • Promote self-esteem.
  • Follow up with referral sources in a timely manner.
  • Always promote a positive image of CHRIS 180 programs.
  • Respond to requests for information from stakeholders in a timely fashion.
  • Answer telephone calls and emails accordingly
  • Specific services to consumers
  • Handle a fast-paced work environment.
  • Maintains a pleasant disposition.
  • Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to consumers.
  • Maintain the voicemail related to CHRIS Counseling Center, updating voicemail outgoing messages, checking, and relating voicemails, returning voicemail requests in a timely manner (within 1 business day). Voicemail should be checked at the beginning of each shift.
  • Reinforce thoughtful decision-making skills.
  • Initiate new consumers into electronic health records.
  • Follow-up calls to referral sources, families, consumers, etc.
  • Coordinating Services and Collaboration
  • Adhere to the organization's policies and procedures.
  • Promote healthy boundaries.
  • Input new consumers into the scheduling system to include insurance information, demographics, referral source and guardian’s telephone number with the date of the referral.
  • Schedule initial Behavioral Health Assessment and initial psychiatric evaluation with consumer and/or guardian
  • Demonstrate exceptional engagement skills in providing trauma-focused mental health treatment to the target population.
  • Reinforce socially acceptable behaviors (ex: manners, eating habits, safety)
  • Collaborate and consult with other providers and entities in the provision of the best care services to consumers.
  • Practice Requirements
  • Make necessary referrals to meet consumers' health, well-being, education, mental health, and employment needs (i.e., Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, employment staffing agencies, local colleges, and tech schools, etc.)
  • Complete Triage forms with new consumers/referral sources/agencies as they are presented and schedule a comprehensive assessment within 3 to 5 business days (36 to72 hours)
  • Demonstrate customer service.
  • Attend and participate in assigned meetings.
  • Respond to consumer, referral, and peer needs in a timely fashion.
  • Ability to answer with a multi-phone system. Calls will be answered by the third ring and the consumer/call will be greeted with “Good Morning, Good Afternoon, Good Evening.”
  • Work as liaison with Utilization Manager
  • Check the BHL web portal daily for any new referrals.
  • Work as a contributing and collaborative team member
  • Complete insurance verifications daily
  • Verify and update patient information in Carelogic.
  • Ensure that 48- and 24-hour phone confirmations for appointments are made each day.
Performance and Conduct
  • Follow the CHRIS 180 Code of Ethics
  • Report changes in status to HR as required by policy.
  • Attend and participate in staff meetings as scheduled.
  • Maintain required certifications.
  • Maintain confidentiality of consumer information.
  • Review, understand, and be responsible for program policies.
  • Provide documents in a timely fashion to Human Resources.
  • Support and maintain an environment of emotional and physical safety for consumers, staff, and other stakeholders always.
  • Maintain appropriate professional boundaries.
  • Follow mandated reporting requirements of GA Code.
  • Follow mandated requirements of accreditation and governing bodies of CHRIS 180.
  • Maintain Personal Information and Training.
  • Attend and participate in required training as scheduled.
Qualifications/Competencies
  • Proficient in MS Office.
  • The ability to learn new programs quickly is also necessary for this position.
  • Ability to recognize when assistance and consultation are needed.
  • Ability to work independently as well as part of a team.
  • Be guided by precedent and established policy and procedure in decision-making. This person will be required to analyze and react to difficult situations, often while under considerable stress and pressure.
  • Self-motivated and able to work under demand and deadlines.
  • Be the backup for all the Front Desk Receptionists throughout the organization when needed.
  • An understanding of the concepts of childcare & mental health/substance abuse disorders
  • Solid decision-making ability as related to the safety and well-being of consumers, families, community, and staff.
  • Organizational skills, excellent attention to detail, and the ability to perform multiple tasks successfully.
  • Ability to effectively relate to consumers, parents/custodians of all social, economic, ethnic, and educational backgrounds, with firmness, fairness, and concern,
  • A minimum of a High School Diploma or GED with two years of employment experience. Preferred qualifications include bilingual abilities in Spanish to assist in catering to the Hispanic population and needs.
  • Dependability, emotional stability, and physical capability. This position requires some physical functions that are essential to satisfactory performance of the outlined job responsibilities, i.e., sitting, standing, walking, driving, speaking, hearing, and the ability to physically restrain (when necessary).

Schedule:
This is a full-time position and typically works Monday – Friday 8:00 am – 5:00 pm but requires flexibility.
