Job Summary:
EvergreenHealth is looking for a team player ready and willing to hop in and support our Nurse Case Management team. Specifically, this role will be pivotal in helping our team with insurance/payor communications, sending clinical information and providing general administrative support to our Care Management Department. This opportunity is a great way to get "behind the scenes" healthcare experience. Our ideal candidate is reliable, highly organized, and a strong communicator.
Primary Duties:
1. Coordinates care and discharge activities including post-discharge care facilities, home care and other community resources as directed by the Care Management clinicians.
2. Provides administrative support including word processing, data entry, filing, organizing and medical record management. Receives direct phone calls and/or voice mail messages and follow-up as appropriate.
3. Collects, copies, and transmits pertinent clinical and patient demographic information required to complete arrangements for post-discharge care and/or placement.
4. Ensures Durable Medical Equipment is arranged as identified by healthcare team.
5. Fields, triages, organizes, distributes other information requests.
6. Responds to requests regarding utilization management, including medical review agencies, insurance companies, healthcare team and community resources.
7. Proactively seek opportunities to improve abstraction and reporting efficiencies and identity non-clinical tasks within the scope of the role.
8. Work within multiple EMR and support tool systems and maintains confidentiality.
9. Practices fiscal stewardship in the role to ensure resources are appropriately utilized.
10. Completes special projects as assigned within scope of training and experience with direction and assistance of the Director, Manager or designee.
11. Performs other duties as assigned.
License, Certification, Education or Experience:
-High school graduation or equivalent
- One (1) year of progressively responsible experience in office or general administrative work
- Ability to multi-task and work well under stress
- Possesses excellent organizational skills and attention to detail
- Excellent customer service
- Excellent communication skills
- Establish priorities and organize work accordingly
- Current Healthcare Provider BLS certification by date of hire
- Ability to function independently
- Ability to communicate effectively and process information regarding patients, internal and external customers
DESIRED for the position:
-Associates degree
- One (1) year experience in a medical setting
- Knowledge of community resources and referral process
- Familiarity with medical terminology
- Successful completion of a secretarial or equivalent program
