We are seeking experienced Care Coordinators to join our extraordinary team!
As a Care Coordinator you will:
- Schedules appointments and performs reminders (via telephone calls, home visits and visits to other community-based organizations, as appropriate) to ensure appropriate resources are available to attend appointments.
- Collaborates regularly with case managers to identify rising risk patients and patient situations that require intervention.
- Navigates referrals to CTO care team to facilitate the support of patient care needs such as transportation, insurance, housing, food, and other community resources according to primary care provider to include attaining or completing applications for such services.
- Completes previsit planning prior to patient’s appointments with primary care providers to ensure gaps in care are identified and addressed.
- Reaches out to high risk patients as identified through program stratification to ensure any social determinants of health (SDOH) are identified and addressed prior to or during patient’s office visit with primary care provider.
- Leverages local agencies throughout the community to assist our patients with getting referrals for the services they need (i.e. Mental Health Providers, Specialists, Radiology Services, Insurance Carriers, etc.).
- Collaborates with primary office and case manager to develop a plan of care to reduce hospital visits.
- Works to prevent unnecessary emergency visits and hospital admissions by communicating with local hospitals, acute and subacute rehabilitation facilities to obtain medical discharge summaries.
- Assists patient and families to identify social, medical and financial needs and barriers.

Qualifications:
- Knowledge of medical and insurance terminology and navigating the healthcare system required
- Previous electronic medical record experience preferred.
- Two (2) years of experience in a medical office setting or healthcare environment required.
- High school diploma or equivalent is required. Associate’s degree preferred.
