Job Description

Job Summary
The Care Coordinator serves a vital member-facing role in problem resolution, escalating issues to the appropriate department when necessary. This is a non-clinical position that requires excellent verbal and written communication skills with an ability to connect with individuals. The candidate must be a team player with a positive attitude and a desire to make a difference in the industry.
Primary Responsibilities
  • Provide non-clinical support for Care Management.
  • Able to establish and maintain strong working relationships both internally and externally.
  • Ensure all job responsibilities are carried out in compliance with the company and State and Federal regulations.
  • Document daily calls in software and follow clinical operation guidelines and polices which include, but are not limited to: documentation of problems, interventions and outcomes
  • Filter and escalate calls or issues to the appropriate department when necessary
Required
  • High School Diploma
Preferred
  • Bachelor’s Degree
Required
  • Strong written and verbal communication skills
  • Proficiency with Microsoft applications (Word, Excel, PowerPoint, Outlook)
  • Strong multi-tasking abilities
  • Minimum of 2 years of customer service experience
