Behavioral Health Support Specialist The Behavioral Health Support Specialist will provide individualized support to patients, couples, and/or families through intake, scheduling, and coordination of referrals for all patients/families. The Behavioral Health Care Support Specialist will function as a core member of the medical home team and is responsible for supporting behavioral health care for individuals and their families with mental, emotional, or substance use problems. Activities may include screening, intake, scheduling, referral management, client advocacy, crisis, and education.
