Job Description

Are you a passionate advanced practice provider ready to serve the community and make a difference in patients' lives? Consider joining a committed, talented group of healthcare professionals at Minnesota Community Care. We are looking for experienced pediatric, family or adult nurse practitioner or physician assistant to help in our School Based Clinics. This role is in a school clinic, and the scheduled hours are during the 42-week school year. There might be opportunity for hours during school breaks, based on availability.


Responsibilities

  • Utilize appropriate and effective patient care plans including adequate diagnosis, treatments, consultations and outcomes
  • Know, understand and adhere to organizational policy related to the patient's rights for confidential care
  • Provide appropriate interpersonal care which includes introducing oneself; treating patients with care, compassion, and courtesy; responding in a non-judgmental and culturally sensitive manner; listening to and answering questions from patients; and clearly explaining diagnoses and treatments
  • Demonstrate effective, culturally sensitive communication skills and effectively communicate verbally and in writing with a variety of people
  • Actively participate and works positively, flexibly and cooperatively in a team effort within and across departments to accomplish the goals of the organization. Willingly assist in areas other than that of primary responsibility for the good of the organization
  • Provide whole person care by using appropriate referrals to the social worker, psychologist, psychiatrist, integrating social and structural determinants of health, including historical trauma, structural racism and biological, social, psychological and environmental factors
  • Manage patients to assure continuity of care including adequate scheduling of return visits, completion of treatment plans and follow-up of chronic problems, tests and no-shows
  • Seek to understand and meet the needs of customers (patients, families, vendors, co-workers, etc.) through respectful, courteous and culturally sensitive interactions

Work Environment and Physical Demands

This job operates in a healthcare setting. This role may occasionally come into contact with patients who may have contagious illnesses. This position is very active and requires standing, and/or walking, bending, kneeling, and stooping. The employee may need to lift and/or move items over 20 pounds.

Advance Practice Registered Nurse

  • Prescription writing privileges
  • Proof of COVID Vaccination Required, Religious and Medical exemption will be considered
  • Practice, Pediatric or Adult Nurse Practitioner or Certified Nurse Midwifery programs.)
  • Responsibility for continuing education credits to maintain licensure.
  • DEA certification
  • Basic Life Support certification.
  • Master's degree, or higher, is required for practice as an Advance Practice Registered Nurse (including Family, Pediatric and Adult)
  • Completion/Certification of an approved Nurse Practitioner program and nationally approved Nurse Practitioner Certification Board is required (i.e. Adult Nurse Practitioner; Family Nurse Practitioner; Women's Health Nurse Practitioner; Pediatric Nurse Practitioner.)
  • Bilingual verbal and written skills in language(s) of primary target population(s) required; Spanish and/or Hmong – PREFERRED.
  • Minnesota Board of Nursing RN licensure, as well as Advance Practice Registered Nurse, for Advanced Practice Nurses.


Physician Assistants

  • Master’s degree, or higher, is required for practice as a Physician Assistant Studies.
  • Completion of an approved study program is required for practice as a Physician Assistant. With Certification through NCCPA. Licensed to practice in Minnesota as a Physicians Assistant. Completion of an approved study program is required for practice as a Women's Health Nurse Practitioner or Physician Assistant.
  • Bilingual verbal and written skills in language(s) of primary target population(s) required; Spanish and/or Hmong – PREFERRED.
  • DEA certification
  • Proof of COVID Vaccination Required, Religious and Medical exemption will be considered
  • Responsibility for continuing education credits to maintain licensure.
  • Prescription writing privileges
  • Basic Life Support certification.


Experience:

  • Minimum one year Advance Practice Registered Nurse/Physician Assistant experience with school ages youth and adolescent health and/or GYN preferred.
